RESOURCE COORDINATION (RC) & CARE MANAGEMENT 30% OF OUR TOTAL ORGANIZATIONAL INCOME IN FY20 CAME THROUGH RESOURCE COORDINATION. WE HELPED OUR CLIENTS ACCESS OVER 875,000 IN SERVICES WITH APPROXIMATELY 4,500 CLIENT CONTACTS. WE PROVIDE INFORMATION & ASSISTANCE, HOUSING AND HOME REPAIR, APPLICATIONS ASSISTANCE (SNAP, FOR EXAMPLE), HOMEBOUND FOOD DELIVERY AND SERVICE COORDINATION TO CLIENTS. THIS TEAM INCLUDES PROFESSIONAL SOCIAL WORKERS, AGING SERVICE SPECIALISTS, TWO SERVICE COORDINATORS AT HUD HOUSING. SNAP OUTREACH TO OVER 15,000 INDIVIDUALS. WE MADE MORE THAN 1,000 COMMUNITY REFERRALS IN RESOURCE COORDINATION. OUR SPECIALIZED CARE MANAGEMENT TEAM INCLUDES A CAREGIVER SUPPORT SPECIALIST AND ELDER JUSTICE NAVIGATOR. CARE MANAGEMENT INCOME WAS 13.7% OF OUR OVERALL REVENUE. OVER 100 CLIENTS WERE SERVED BY OUR ELDER JUSTICE NAVIGATOR. OVER 300 CAREGIVERS RECEIVED RESPITE AND SUPPORT. OUR SUPPORT GROUP SERVED OVER 20 INDIVIDUALS.
SENIORS SAFE AT HOME THE AFFORDABLE CARE ACT (ACA) PROGRAM HAD 336 APPOINTMENTS AND ENROLLED 172 INDIVIDUALS IN EIGHT WNC COUNTIES. SENIORS' HEALTH INSURANCE AND INFORMATION (SHIIP) HAD 3,922 CLIENT CONTACTS AND 39 MEDICARE CLASSES IN BOTH BUNCOMBE AND HENDERSON COUNTIES. THROUGH THE NCOA BENEFITS ENROLLMENT CENTER, WE HAD 394 LOW-INCOME MEDICARE BENEFICIARIES RECEIVE ENROLLMENT ASSISTANCE WITH A VALUE OF BENEFITS RECEIVED = 1,100,384. THESE INSURANCE SERVICES ACCOUNT FOR 22.4% OF OUR REVENUE. IN ADDITION TO THE ABOVE, WE OFFER MINOR HOME REPAIR AND VOLUNTEER TRANSPORTATION SERVICES (CALL A RIDE/CAR). IN MINOR HOME REPAIR, WE PROVIDE CONTRACT SERVICES THROUGH PARTNERSHIPS IN OUR AREA (MAHEC HOME BASED PRIMARY CARE AND PACE) AS WELL AS OFFER ASSISTANCE THROUGH RC AS PART OF OUR WRAPAROUND SUPPORT. WE ASSIST WITH GRAB BARS AND OTHER MINOR HOME IMPROVEMENTS. IN CAR, WE PROVIDE VOLUNTEERS WHO ASSIST INDIVIDUALS IN GETTING TO/FROM APPOINTMENTS AND ON ERRANDS AS PART OF THEIR ACCESS TO SERVICE AND SUPPORT.
HEALTH AND WELLNESS THE SENIOR DINING AND WELLNESS PROGRAM (16.9% OF FY20 REVENUE) PROVIDES NUTRITIOUS MEALS THAT MEET RECOMMENDED DAILY ALLOWANCE (RDA) FOR OLDER ADULTS AS WELL AS NUTRITION EDUCATION AND ACTIVITIES TO KEEP OLDER ADULTS ENGAGED. APPROXIMATELY 420 MEALS PER WEEK WERE SERVED TO SENIOR DINING CLIENTS, 5,209 OF WHICH WERE SERVED DURING THE PANDEMIC.
