THE NETWORK HAS A SHARED VISION, PURPOSE, PRIORITIES, STRATEGIES, AND OBJECTIVES ALIGNED TO IMPROVE THE HEALTH OF UNDER RESOURCED PEOPLE AND COMMUNITIES. CARE SHARE'S STATEWIDE TECHNICAL ASSISTANCE SERVICES HELPS COMMUNITIES IMPROVE HEALTH BY: 1) LEVERAGING NEW AND EXISTING RESOURSES. 2) INCREASING PHYSICIAN VOLUNTEERS DONATING CARE. 3) INCREASING ACCESS TO HEALTHCARE, SOCIAL AND COMMUNITY RESOURCES. 4) DEVELOPING COMMON REFERRAL NETWORKS ALL PROVIDERS CAN USE. 5) EXPANDING THE CONTINUUM OF CARE IN LOCAL COMMUNITIES. 6) HELPING NETWORKS CREATE EFFICIENT SYSTE
SINCE 2009, PROVIDED ACTIVE TECHNICAL ASSISTANCE TO 19 COLLABORATIVE NETWORKS COVERING 52 OF 100 COUNTIES IN NORTH CAROLINA. PROVIDED SERVICES ON-SITE AND PROVIDED ON-LINE GUIDANCE TO HELP COMMUNITIES COORDINATE CARE AROUND TOPICS SUCH AS ORGANIZATIONAL DEVELOPMENT, PROGRAM DESIGN, IT ASSESSMENT, LEADERSHIP COACHING FOR DIRECTORS/COORDINATORS, FACILITATING PEER-TO-PEER LEARNING ACROSS NETWORKS AND MODELS, WEBINARS, BUSINESS PLAN DEVELOPMENT AND COLLABORATIVE FUNDNG PROPOSAL DEVELOPMENT. SUPPORTED THE DEVELOPMENT OF COLLABORATIVE MODELS LIKE THE COMMUNITY CENTERED HEALTH, NC GET COVERED, AND LOCAL FOOD COUNCILS. SUPPORTED NETWORKS AND COALITIONS TO HELP THEM INCREASE COLLABORATION IN THEIR COMMUNITIES, REVIEW STRATEGIC PLANS, AND STRENGTHEN EXISTING NETWORKS AND COALITIONS. COLLECTED AND DISSEMINATED BEST PRACTICES FOR COLLABORATIVE NETWORKS.
