Organizations Filed Purposes: TO PROMOTE HEALTH FOR THE BENEFIT OF THE COMMUNITY BY IDENTIFYING AND SUPPORTING HEALTHCARE INNOVATION AND COLLABORATION TO ACHIEVE IMPROVED QUALITY, REDUCED COSTS, AND IMPROVED PATIENT EXPERIENCE.

THE COHQA GRANT PROGRAM: THE GRANT PROGRAM IS FUNDED THROUGH THE SURPLUS GENERATED BY PARTNER ORGANIZATION, THE CENTRAL OREGON INDEPENDENT PRACTICE ASSOCIATION'S (COIPA) OREGON HEALTH PLAN CONTRACT WITH PACIFICSOURCE. THE FUNDS ARE OFFERED AT UP TO 30,000 PER YEAR FOR PROJECTS ADDRESSING THE FOLLOWING PRIORITIES: 1) PROVIDER-LED HEALTH CARE INNOVATION: GOAL TO IMPLEMENT INTERVENTION(S) SPECIFIC TO HEALTH CARE DELIVERY SYSTEMS, CLINICIANS, AND/OR PATIENTS THAT FOCUS ON THE TRIPLE AIM OF: IMPROVED QUALITY, REDUCED COST, AND IMPROVED PATIENT EXPERIENCE IN THE COMMUNITIES THAT COIPA SERVES. CONTINUED ON SCHEDULE O. 2) CLINICAL INTEGRATION: IMPLEMENTATION OF INTERVENTION(S) THAT ADVANCE CLINICAL AND FINANCIAL COORDINATION AND INTEGRATION OF STAKEHOLDERS IN THE COMMUNITIES WE SERVE. 3) SKILL TRANSFER: EFFICIENTLY AND EFFECTIVELY TO TRANSFER SKILLS AND LEARNED PROCESSES TO OTHER COIPA MEMBERS AND THEIR CLINICS IN A COLLABORATIVE FASION. 4) COMMUNITY ENGAGEMENT: PARTNERSHIP WITH COMMUNITY MEMBERS AND ORGANIZATIONS OUTSIDE THE HEALTH CARE INDUSTRY TO ENGENDER POPULATION-LEVEL PUBLIC HEALTH IMPROVEMENTS, AIMING TO INCREASE COMMUNITY SAY IN AND OWNERSHIP OF HEALTH AND HEALTH CARE.

COHQA GRANT-FUNDED PROJECTS: COHQA WILL SEEK TO APPLY FOR GRANTS TO FUND LARGER, MULTI-PARTNER PROJECTS IN THE COMMUNITY, AND IF AWARDED, ADMINISTER AND PERFORM THE SPECIFIED ACTIVITIES TO SUCCESSFULLY MEET THE GOALS OF THE AWARDED GRANTS. COHQA WILL SPECIFICALLY TARGET GRANT OPPORTUNITIES THAT SPEAK TO OUR UNIQUE STRENGTHS AS A BRIDGE ACROSS MANY DISPARATE ACTORS IN THE HEALTH CARE COMMUNITY IN CENTRAL OREGON. THIS WILL ALLOW US TO DEVELOP, IMPLEMENT, AND EVALUATE INNOVATIVE NEW HEALTH CARE DELIVERY SYSTEMS AND TRANSFORMATIVE QUALITY IMPROVEMENT IDEAS THAT REQUIRE COORDINARTION ACROSS PROVIDERS, HEALTH ORGANIZATIONS, AND COMMUNITIES TO SUCCESSFULLY IMPLEMENT. CONTINUED ON SCHEDULE O. PARTICULAR FOCUS WILL BE ON 1) IMPROVING COORDINATION OF CARE ACROSS PATIENTS' ENTIRE CARE TEAM, 2) INCREAING PATIENT KNOWLEDGE OF AND ENGAGEMENT IN THEIR TREATMENT AND HEALTH CARE OPTIONS, AND 3) EXPLORING ALTERNATIVE PAYMENT MODELS THAT CAN BETTER ALIGN INCENTIVES AMONG PATIENTS, PROVIDERS, AND INSURERS.
