Organizations Filed Purposes: THE FOUNDATION'S MISSION IS TO PROVIDE PHILANTHROPIC RESOURCES FOR GOOD SAMARITAN HOSPITAL CORVALLIS AND ITS CHARITABLE PURPOSES IN ORDER TO PROMOTE STATE-OF-THE-ART HEALTHCARE, HEALTH EDUCATION, AND WELLNESS.

TO SUPPORT GOOD SAMARITAN HOSPITAL, CORVALLIS AND SERVE THE COMMUNITY.

GRANTS TO HOSPITAL AND SUPPORTING ORGANIZATIONS OR PROGRAMS FOR OPERATING AND CAPITAL COSTS.GRANTS FOR CAPITAL COSTS DURING 2019 INCLUDED FUNDING FOR HOSPITAL EQUIPMENT THAT FACILITATES STATE-OF-THE-ART CANCER TREATMENT, NEW SLEEPER CHAIRS FOR THE LABOR AND DELIVERY UNIT, AND FURNITURE FOR THE SAMARITAN STUDENT HEALTH CENTER AT THE NEW BOYS AND GIRLS CLUB IN CORVALLIS, OREGON. THERE WERE CONSTRUCTION COSTS FOR REMODELING THE LABOR AND DELIVERY BATHROOMS, AND FOR THE RECENTLY BUILT CANCER CENTER OFFERING EXPANDED CANCER SERVICES. THE FILING ORGANIZATION JOINED TWO RELATED ORGANIZATIONS TO SUPPORT THE BUILDING OF THE SAMARITAN TREATMENT AND RECOVERY SERVICES BUILDING LOCATED IN LEBANON, OR. THE 16-BED RESIDENTIAL TREATMENT FACILITY, WHICH WILL OPEN IN AUGUST 2020, IS DESIGNED AS A SAFE HARBOR THAT WILL SERVE ADULTS 18 AND OLDER WITH SERIOUS SUBSTANCE USE DISORDERS THROUGHOUT OREGON. PRIORITY WILL BE GIVEN TO PATIENTS WHO LIVE IN BENTON, LINCOLN AND LINN COUNTIES.PROGRAM GRANTS IN 2019 INCLUDED FUNDING FOR HOSPITAL PROGRAMS, SUCH AS MENTAL HEALTH SERVICES , CANCER CARE AND CANCER SURVIVOR SUPPORT SERVICES, STAFF EDUCATION, A PREGNANCY CENTERING GROUP, A TOBACCO CESSATION GROUP, AN ARTSCARE PROGRAM FOR PATIENTS AND STAFF, AND SUPPORT FOR THE MARIO PASTEGA GUEST HOUSE LOCATED ON THE HOSPITAL CAMPUS. THE GUEST HOUSE PROVIDES A "HOME AWAY FROM HOME" FOR PATIENTS AND FAMILIES TRAVELING TO CORVALLIS FOR SPECIALIZED MEDICAL CARE. OTHER GRANTS DURING 2019 INCLUDED FUNDING FOR RESEARCH GRANTS WHICH SUPPORT PROGRAMS/PROJECTS THAT ENHANCE HUMAN FUNCTION AND PERFORMANCE THROUGH CLINICAL RESEARCH, REHABILITATION, AND/OR PREVENTION. SCHOLARSHIPS FOR NURSING AND HEALTHCARE ADMINISTRATION EDUCATION. SEE SCHEDULE I, PART II FOR A LISTING OF 2019 GRANTS.

PROVIDED EMERGENCY FINANCIAL ASSISTANCE TO HOSPITAL EMPLOYEES WHO ARE EXPERIENCING AN UNEXPECTED FINANCIAL NEED, THE CAUSE OF WHICH IS BEYOND THE EMPLOYEE'S CONTROL. ACCEPTABLE FUNDING REQUESTS MAY INCLUDE: EVICTION NOTIFICATION, UTILITY TURN-OFF NOTIFICATIONS, PAST DUE MEDICAL BILLS, AND UNEXPECTED/UNPLANNED EXPENSES. SEE SCHEDULE I, PART III.

SCHOLARSHIPS PROVIDED FOR STUDENTS CURRENTLY PURSUING AN UNDERGRADUATE OR GRADUATE DEGREE IN HEALTH CARE RELATED FIELD. SCHOLARSHIPS ARE AVAILABLE TO INDIVIDUALS WITHOUT REGARD TO RACE, RELIGION, NATIONAL ORIGIN, GENDER, AGE, COLOR, DISABILITY, OR MARITAL STATUS. SPECIAL CONSIDERATION IS GIVEN TO APPLICANTS WHO ARE CURRENTLY EMPLOYED IN THE HEALTH CARE FIELD AND TO MEMBERS OR VETERANS OF THE U.S. MILITARY. SEE SCHEDULE I, PART III.
