To Promote Health and Improve wellness among Oregon's African American community through health education, advocacy and research.
EDUCATION AND OUTREACH: SUPPORT THE AFRICAN AMERICAN COMMUNITY WITH EVIDENCE BASED, COMMUNITY APPROACHES TO CHRONIC DISEASE PREVENTION AND CONTROL. THESE SERVICES INCLUDE A CHRONIC DISEASE SELF-MANAGEMENT PROGRAM AND A DIABETES PREVENTION PROGRAM. BOTH INCLUDE EXTENSIVE LEARNING OPPORTUNITIES AND FOLLOWUP.
EXERCISE PROGRAM: ONGOING PHYSICAL ACTIVITY PROGRAM TO PROVIDE AFRICAN AMERICANS ACCESS TO CHARLES JORDAN AND MATT DISHMAN COMMUNITY CENTERS INCLUDING POOLS. PARTICIPANTS OF THE PROGRAM GET THE BENEFITS FO DAILY USE OF THE FACILITY FOR A MINIMAL QUARTERLY FEE.
