Organizations Filed Purposes: WORKING TO MEET THE HEALTH AND WELLBEING NEEDS OF THE UNDERSERVED IN OUR COMMUNITY THROUGH PROJECT ACCESS, MEDASSIST, WOMEN'S HEALTH PROGRAM AND MOMS PLUS.

WOMEN'S HEALTH PROGRAM WAS ACQUIRED IN 2013 FROM ANOTHER LOCAL NON-PROFIT. IN THE LAST YEAR, WE HAVE WORKED WITH OVER 100 WOMEN WHO WERE CONCERNED ABOUT THEIR BREAST HEALTH THROUGH OUR HEALTH PARTNERS WITH CLIENT NAVIGATION AND MAMMOGRAM VOUCHERS. IF WOMEN NEED FURTHER DIAGNOSTIC TESTING, OUR FUNDRAISING PROVIDE FOR SCREENING MAMMOGRAMS FOR WOMEN WHO ARE UNDER INSURED AND WHO DON'T QUALIFY FOR THE EXPANDED OREGON HEALTH PLAN. IF FURTHER DIAGNOSTIC TESTING IS NEEDED, OUR PROGRAMS PROVIDE FOR THOSE SERVICES FREE. IN OUR EFFORTS TO CONTINUE TO BUILD AWARENESS OF THE IMPORTANCE OF BREAST HEALTH WE ARE PROUD TO PARTNER WITH SUSAN G. KOMEN FOUNDATION TO EXPAND OUR WOMEN'S HEALTH PROGRAM WITH THE LATINA WOMEN INITIATIVE LAST YEAR TO ADDRESS THE HIGH RATE OF LATE STAGE BREAST CANCER DIAGNOSIS. WE ALSO CONTINUE OUR WORK IN EDUCATING AND REACHING OUT TO WOMEN IN OUR COMMUNITIES ABOUT THE IMPORTANCE OF EARLY DETECTION AND BREAKING DOWN THE CULTURAL AND FINANCIAL BARRIERS THAT OFTEN KEEPS WOMEN FROM SEEKING OUT THIS LIFE SAVING SCREENING. WE CONTINUE TO HOST THE LOCAL BREAST CANCER SUPPORT GROUP WHO HAS BEEN TOGETHER FOR ALMOST 20 YEARS. ELIGABLE WOMEN ARE SCREENED FOR BREAST CANCER, AND WE PROVIDED VOUCHERS AS NEEDED FOR FURTHER FOLLOW UP AND LAB WORK. AS PART OF OUR LATINA INITIATIVE WE PROVIDED MAMMOGRAMS TO WOMEN WHO HAD NOT HAD AN EXAM IN THE PAST FOUR YEARS.

MOMS PLUS PROVIDES FREE PROFESSIONAL CERTIFIED PEER TO PEER MENTORSHIP FOR AT RISK AND RECOVERING MOMS WHO ARE IN RECOVERY AND NO LONGER ELIGIBLE FOR THE PROGRAM THROUGH THE OREGON HEALTH PLAN. THE PROGRAMS GOAL IS TO PROVIDE THE MOMS A SAFE, NON-THREATENING AND AUTHORITY FREE SUPPORT SYSTEM WHILE TEACHING THE MOMS SELF SUFFICIENCY SKILLS NEEDED TO MOVE FORWARD WITH THEIR FAMILIES. OUR PROGRAM PROVIDES UP TO 18 MONTHS OF FREE CERTIFIED PEER SUPPORT SPECIALISTS TRAINED TO WORK WITH THE MOMS IN A ONE ON ONE SETTING. SINCE AUGUST 2013 OUR CERTIFIED MENTORS HAVE HELPED NEARLY 100 MOMS ON THEIR PATH TO SOBRIETY, OVERCOMING CHALLENGING SITUATIONS AND BUILDING SELF SUFFICIENCY SKILLS TO KEEP THEIR FAMILIES TOGETHER.

THE FOUNDATION OPERATED A MEDASSIST PROGRAM, WHICH PROVIDES ELIGIBLE LOW- INCOME INDIVIDUALS ACCESS TO FREE PRESCRIPTION MEDICATIONS. THE FOUNDATION'S COMPANION PROGRAM WAS PROJECT ACCESS. PROJECT ACCESS COORDINATED, AT NO CHARGE TO ELIGIBLE LOW-INCOME INDIVIDUALS, A FULL SPECTRUM OF HEALTH CARE SERVICES THROUGH AS MANY AS 250 PARTICIPATING PROVIDERS WHO DONATE ACCESS TO MEDICAL CARE.
