Youth and Family Services Program:Youth and Family Services provides a continuum of support to youth with disabilities, their families, and unaccompanied youth, assisting them to identify goals and create a plan of how to achieve them. Support activities across programs include assessing needs and strengths, identifying desired outcomes, making referrals, attending related meetings with clients, facilitating a team-based process and plan of care, engaging collaboratively with other service providers, assisting clients with applications for services, stabilizing education plans, and engaging natural supports. Programs include Service Coordination, Comprehensive Advocacy and Planning, Youth Advocacy Program, and Wraparound.Assisted 237 new clients across programs and 259 continuing clients for a total of 496 youth with special needs and disabilities served.
Direction Service Counseling Center:Provided outpatient mental health services to 590 clients, including 494 adults and 96 children from 7/1/2019 through 6/30/2020.
