OUR MEDICARE/MEDICAID LONG-TERM CARE PROGRAMS SERVE THE NEEDS OF OLDER ADULTS AND ADULTS WITH DISABILITIES. WE COORDINATE AND DELIVER A FULL RANGE OF SUPPORTIVE SERVICES THAT HELP THEM LIVE SAFELY, CONFIDENTLY AND WITH DIGNITY.
THE COMMUNITY CARE PACE PROGRAM IS THE ONLY PACE PLAN IN THE STATE OF WISCONSIN AND HAS BEEN IN OPERATION FOR MORE THAN 25 YEARS. COMMUNITY CARE PACE CURRENTLY SERVES MORE THAN 550 MEMBERS IN MILWAUKEE, WAUKESHA AND RACINE COUNTIES.PACE OFFERS A BENEFIT PACKAGE THAT COMBINES MEDICAL CARE, LONG-TERM CARE AND PRESCRIPTION DRUGS. PACE HELPS FRAIL AND DISABLED INDIVIDUALS AGE 55 AND OLDER LIVE AS INDEPENDENTLY AS POSSIBLE FOR AS LONG AS POSSIBLE WITHIN THE COMMUNITY.
COMMUNITY CARE'S FAMILY CARE PARTNERSHIP PROGRAM FOR FRAIL ELDERS AND ADULTS WITH PHYSICAL, INTELLECTUAL OR DEVELOPMENTAL DISABILITIES PROVIDES COMPREHENSIVE MEDICAL AND LONG-TERM CARE.PARTNERSHIP MEMBERS HAVE A TEAM OF PROFESSIONALS THAT WORK TO COORDINATE ALL ASPECTS OF CARE. THE TEAM WILL ASSESS MEMBER NEEDS AND DEVELOP A COST-EFFECTIVE CARE PLAN. PARTNERSHIP MEMBERS HAVE ACCESS TO MEDICAL CARE AND LONG-TERM CARE SERVICES WITHOUT THE WORRY OF INSURANCE BILLS, CO-PAYS, DEDUCTIBLE OR INSURANCE GAPS. AS OF THE END OF 2019 PARTNERSHIP SERVES 681 MEMBERS IN NINE WISCONSIN COUNTIES.
