CLC IS A LOCAL, NON-PROFIT, SERVICE ORIENTED ORGANIZATION WHOSE MISSION IS TO PROVIDE THE HIGHEST QUALITY RESIDENTIAL SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES. WE ENSURE PERSON CENTERED SUPPORTS THAT EMPHASIZE DIGNITY, RESPECT AND INDEPENDENCE. OUR SERVICES AIM TO CONNECT INDIVIDUALS WITH THEIR COMMUNITY AND HELP EACH PERSON REACH THEIR FULLEST POTENTIAL.
OUR COMMUNITY LIVING PROGRAMS SUPPORT INDEPENDENT LIVING ENVIRONMENTS FOR ADULTS WITH DEVELOPMENTAL DISABILITIES. WE ENSURE PERSON CENTERED SUPPORTS THAT EMPHASIZE DIGNITY, RESPECT AND INDEPENDENCE. OUR SERVICES AIM TO CONNECT INDIVIDUALS WITH THEIR COMMUNITY AND HELP EACH PERSON REACH THEIR FULLEST POTENTIAL. DURING 2019, COMMUNITY LIVING CONNECTIONS PROVIDED SERVICES TO APPROXIMATELY 187 INDIVIDUALS WITH VARYING NEEDS AND VARYING DEGREES OF SUPPORT.1. RESIDENTIAL SERVICES: AT THE END OF2019 CLC SUPPORTED 15 INDIVIDUALS IN 4 HOMES LICENSED BY THE STATE OF WISCONSIN AS ADULT FAMILY HOMES. THESE HOMES ARE STAFFED BETWEEN 18-24 HOURS PER DAY, OFTEN WITH MULTIPLE STAFF WORKING.2. GENERAL SERVICES: AT THE END OF 2019 CLC SUPPORTED 165INDIVIDUALS; SERVICES PROVIDED RANGE FROM A FEW HOURS A WEEK TO 24 HOURS PER DAY PER INDIVIDUAL. UNDER THIS SERVICE CATEGORY, INDIVIDUALS HAVE A DAY RATE SET BY MCO'S OR FEA'S.3. CCA: AT THE END OF2019 CLC SERVED 24 INDIVIDUALS UNDER THIS DAY SERVICES CATEGORY.4. PRIVATE PAY CONTRACTS: AT THE END OF2019 CLC SUPPORTED 4 PEOPLE THROUGH PRIVATE PAY CONTRACTS.DUE TO THE NATURE OF THE SERVICES THAT WE PROVIDE MANY OF THE OUTCOMES WE STRIVE TO ACHIEVE ARE INDIVIDUALIZED (BY EACH CONSUMER AND THEIR TEAMS) AND THEY ARE NOT MEASURABLE USING CONVENTIONAL INDICATORS. SOME OF THE ACHIEVEMENTS THAT ARE DIFFICULT TO MEASURE BUT NO LESS CRITICAL TO OUR MISSION ARE: * COMMUNITY INCLUSION WHICH BENEFITS THE PEOPLE WE SERVE AND OUR ENTIRE COMMUNITY BY PROMOTING DIVERSITY.* AUTONOMY AND CHOICE, INDIVIDUALS HAVING THE RIGHT TO DIRECT THEIR OWN LIVES* RELATIONSHIPS* PERSONAL SAFETY AND FREEDOM FROM ABUSE* ACCESS TO HIGH QUALITY HEALTH CARE AND SUPPORT AROUND HEALTH AND WELLNESS* DIGNITY, PRIVACY AND RESPECT BOTH WITHIN CLC AND THE COMMUNITY AT LARGE.
