FAMILY CARE PROGRAM - PROVIDES CARE MANAGEMENT AND MEDICAID BENEFIT AND SERVICE PACKAGE. FOCUS IS ON MEETING MEMBERS DESIRED HEALTH AND LONG-TERM CARE OUTCOMES THROUGH EFFECTIVE AND COST EFFECTIVE CARE PLANS. FAMILY CARE HAD 8,093 MEMBERS ENROLLED AT THE END OF 2019.
PARTNERSHIPS PROGRAM - PROVIDES SERVICES TO CARE WISCONSIN IN HEALTH PLAN, INC. FOR THE PARTNERSHIP PROGRAM. THESE SERVICES INCLUDE COORDINATED HEALTH CARE AND LONG-TERM SUPPORT SERVICES FOR MEDICAID ELIGIBLE MEMBERS WHO HAVE CONDITIONS REQUIRING NURSING HOME LEVEL OF CARE. PARTNERSHIPS HAD 1,919 MEMBERS ENROLLED AT THE END OF 2019.
CARE WISCONSIN FIRST, INC. PROVIDES SERVICES TO CARE WISCONSIN HEALTH PLAN, INC. FOR THE MEDICAID SSI THAT IS OFFERED. THE MEDICAID SSI PROGRAM IS A HEALTH INSURANCE PROGRAM FOR INDIVIDUALS WITH A DISABILITY WHO QUALIFY FOR FEDERAL SSI. THE SSI PROGRAM HAD 2,951 MEMBERS AS OF THE END OF 2019.
