Organizations Filed Purposes: CORNERSTONE RECOVERY AND SUPPORTS (CRS) IS A NON-PROFIT ORGANIZATION WHOSE MISSION IS TO EMPOWER PEOPLE WHO EXPERIENCE MENTAL ILLNESS AND CO-OCCURRING DISORDERS TO ACCESS THE RESOURCES NECESSARY TO IMPROVE HIS/HER QUALITY OF LIFE WHILE MAINTAINING A SAFE LIVING ENVIRONMENT.

COMMUNITY RESIDENTIAL REHABILITATION SERVICES: THIS PROGRAM IS DESIGNED TO OFFER ASSISTANCE TO ADULTS WITH CHRONIC PSYCHIATRIC DISABILITIES. THE SERVICES ARE A GATEWAY FOR PEOPLE TO DEVELOP AND HONE THE SKILLS THAT WILL ALLOW THEM TO LIVE INDEPENDENTLY IN THEIR COMMUNITY OF CHOICE. CORNERSTONE RECOVERY AND SUPPORTS OFFERS COMMUNITY RESIDENTIAL REHABILITATION (CRR) SERVICES IN TWO LOCATIONS WITHIN THE BEAVER COUNTY AREA. THE FIRST LOCATION IS A FULL CARE CRR FOR UP TO 12 ADULTS. SINCE THIS IS A FULL CARE CRR, STAFF WILL BE PRESENT ANY TIME A CONSUMER IS ON SITE. THE SECOND LOCATION IS A PARTIAL CARE CRR FOR UP TO 8 ADULTS. STAFF WILL BE ON SITE ON A REGULARLY SCHEDULED BASIS, INCLUDING EVENINGS AND WEEKENDS. CRS WILL PROVIDE OPPORTUNITIES FOR CONSUMERS TO CONTRIBUTE IN SIGNIFICANT AND MEANINGFUL WAYS. THEY WILL WORK SIDE BY SIDE WITH STAFF IN A PARTNERSHIP. THIS PARTNERSHIP WILL CONSIST OF: - SECURE HOME-LIKE ACCOMMODATIONS - MAINTAIN A CLEAN & SANITARY HOME - SUPPORT IN ACHIEVING THE MAXIMUM LEVEL OF AUTONOMY, INDEPENDENCE AND SELF DETERMINATION - MONEY MANAGEMENT - SELF MEDICATION CRR IS AN INDEPENDENT GUIDANCE PROGRAM FOR ADULTS. THE PROGRAM IS TRANSITIONAL AND LENGTH OF STAY WILL DEPEND ON THE NEEDS OF THE INDIVIDUAL.

SUPPORTED HOUSING: THE SUPPORTED HOUSING PROGRAM IS A PILOT PROGRAM DESIGNED FOR PEOPLE WHO ARE READY FOR INDEPENDENT LIVING, BUT WHO DO NOT WANT TO LIVE ALONE AT THAT TIME. EACH IMPLEMENTATION OF THIS PROGRAM INVOLVES THREE PEOPLE WHO CHOOSE TO LIVE TOGETHER AND ARE PREPARED TO WORK AS A UNIT, WITH SOME ASSISTANCE FROM SUPPORT PERSONNEL, TO MAINTAIN AND MANAGE THEIR HOME. CORNERSTONE RECOVERY AND SUPPORTS OFFERS SUPPORTED HOUSING TO OVER SIXTY CONSUMERS THROUGHOUT THE BEAVER COUNTY AREA. THESE SUPPORTS ARE PROVIDED TO CONSUMERS THAT LIVE INDEPENDENTLY AND MAINTAIN THEIR OWN LEASES. THESE SUPPORTS INCLUDE BUT ARE NOT LIMITED TO: - MAINTAIN HOUSING ELIGIBILITY - ACCESS COMMUNITY RESOURCES - ASSIST WITH TRANSPORTATION - BECOME AN ACTIVE MEMBER OF THE COMMUNITY - BUDGETING AND MONEY MANAGEMENT - UTILIZATION OF APPROPRIATE PSYCHIATRIC SERVICES.

SUPPORT SERVICES FOR CHRONICALLY HOMELESS INDIVIDUALS (STONE HARBOUR): THIS PROGRAM OFFERS APARTMENT-STYLE LIVING FOR PEOPLE WITH A BEHAVIORAL HEALTH DIAGNOSIS THAT ARE ALSO EXPERIENCING HOMELESSNESS, DRUG AND/OR ALCOHOL ADDICTION AND HAVE A LEGAL HISTORY. STONE HARBOUR PROVIDES A SAFE, THERAPEUTIC TRANSITIONAL ENVIRONMENT THAT PROMOTES INDEPENDENCE AND RECOVERY. SUPPORT PERSONNEL OFFER CASE MANAGEMENT SERVICES AS WELL AS OUTREACH SUPPORTS. LENGTH OF STAY MAY NOT EXCEED 24 MONTHS.
