Concordia Community Support Services (CCSS) was formed in the spring of 2015 to provide executive level oversight, management and strategic planning for the non-profit home and community based entities that deliver quality home health services, hospice services, and supply needs in good faith, on behalf of those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity and compassion. CCSS will perform management and policy functions and the organization is committed to providing strong leadership so that the agencies can provide the highest quality of care from nursing and therapy to specialized services such as IV therapy, wound care, palliative care, and psychiatric care, all from the comfort of the patients' home.

Concordia Community Support Services (CCSS) program service accomplishments are a testament to the mission and vision that guides the actions of those we employ, "to serve our aging community with a continuum of high quality caregiving options, provided in a Christian environment, and to serve those with limited funds to the best of our ability". CCSS manages the leadership and operations of Concordia Visiting Nurses; Alliance Visiting Nurses, dba, VNA Alliance; Good Samaritan Hospice of Pittsburgh; Concordia Hospice of Washington, Concordia Private Care, and Concordia Medical Equipment. All of these entities are 501(c)(3) non-profit entities. During the fiscal year, approximately 21,460 patients were served across 15 counties in home health, hospice and private duty services. In addition, Concordia Medical Equipment supports the approximately 40,000 patients that Concordia Lutheran Ministries serves over 12 campuses by providing durable medical equipment and supplies. At CCSS, our faith in God is what motivates us to serve His people. CCSS is part of a large continuum of care striving to succeed with aging in place philosophy by providing a wide breadth of services. From nursing and therapy services to specialized services such as IV Therapy, wound care, palliative care and mental health care, our visiting nurses teams ensure that patients are provided care in the comfort of their own home with the goal of preventing rehospitalization and monitoring chronic disease states. Our hospice team provides comfort care to improve the overall quality of life of patients who are on their end-of-life journey. Our medical equipment agency is dedicated to enhancing home safety by providing a wide selection of durable medical equipment and related home medical equipment and supplies such as home accessibility products, mobility aids, and respiratory products and services. The care provided by the employees of these organizations goes far beyond the healthcare medical services for the patients. Families of our patients play a vital role so we educate and support them through situations related to caring for a loved one who is aging, all the way through their end-of-life. Bereavement care is also provided to families of hospice patients for at least a year beyond the patient's death. The organization is committed to mentoring and teaching; therefore, providing the highest quality of care to not only our patients but also to our employees and the members of the community in which we serve. Employees are offered training and opportunities to obtain new skills such as Wound Care Certification, Palliative Care Training, IV Therapy Certification, Lymphedema Therapy Certification, Home Health and Hospice Coding Certification, and Hospice Boot Camp Training. Employees are offered an opportunity to attend nursing school tuition-free and two employees took advantage of the offer during the fiscal year. The spiritual care provided to our patients is also available to our employees. A Spiritual Care Room has been created in our Cabot office which serves as a comfortable space for meditation, prayer, and counseling with members of the Pastoral team. Our Cabot office conference room is used for a weekly TOPS (Taking Off Pounds Sensibly) meeting which is open to employees and members of the community to offer tools and programs for healthy living and weight management. Our Director of Marketing participates in educational radio shows which are broadcast 1 time a month in Beaver, 2 times a month in Butler, and 1 time per month (for six months of the fiscal year) in the New Castle area. The cost of the time invested by our Director of Marketing is approximately $5,000. The goal of these radio shows is to educate family caregivers, whether they are a senior caring for a spouse or adult children caring for an older adult. Shows often talk about caregiving tips and the listener is directed to the services offered by CCSS companies so they can make informed decisions on care. Some of the topics covered in 2018-2019 include: Age-Friendly Kitchen, BHS Dietrich Symposium 2018, Common Elder Scams, Exercise and COPD 2018, I Don't Need Help (a Three-Part Series), Keeping Your Marbles:Reducing Dementia Risk Living with Pulmonary Disease, Medicare Changes for 2019, Opioid Addiction and Serious Illness, Preparing for Cold and Flu Season 2018, Preparing for Joint Replacement, Talking to Your Doctor about Risks of Falls. In addition to radio shows, our educational websites are helpful to visitors who are in need of family caregiving tips including a wide range of needs from Alzheimer's and memory impairment to common diseases like CHF, COPD and Diabetes. The Palliative care website assists people who struggle with a serious illness but are still seeking curative care. There are two hospice websites, one for the Pittsburgh market and one for the Washington, PA market, which are helpful for people suffering from a serious illness and are needing hospice care. Leadership served at Katie's Kitchen, St. Mark's Lutheran Church in Butler, PA. The kitchen provides a free meal every Thursday. Individuals participate in serving the meal and cleanup afterwards. Several of our CCSS employees and members of management serve annually at Concordia Summer Festival and Concordia Octoberfest. Both events are sponsored by Concordia Lutheran Health and Human Care and are open to the public as a way of providing entertainment, food and refreshments and a feeling of community. One CCSS employee in particular devotes an extra 40 hours per year to coordinate the Kids Area for each festival which includes activities, crafts and face painting, all of which are provided free to the public. A Mission Committee was formed by CCSS to organize opportunities for employees to serve the community. The committee consists of employees of CCSS and the six agencies that we manage and the approximate cost of the time invested in the meetings to organize the events is $7,200. Employees from all our agencies are encouraged to participate. The events completed this year include: A Servant Event at Prince of Peace in Farrel, PA where employees were able to serve food for many families as well as make snow cones, bag popcorn, run a bounce house and help with the clean-up at the block party for community children. St Luke Lutheran Church House Build/Servant Day is an event where employees of Concordia and members of the St Luke congregation built the framing for a dormitory for teens who need a place to live post-foster care age. Give Thanks in a Tote event where thanksgiving meal supplies, a gift card, and an encouraging devotion were placed in a tote and given to nearly 200 patients under our care. Employees volunteered their time to serve meals to 120 individuals at the North Side Homeless Alliance Winter Shelter in Pittsburgh, PA and sort through donations of winter clothes. Employees participated in a Fleece Blanket Mission Event where employees volunteered during their lunch break to cut and tie fleece blankets that were donated to families at the Ronald McDonald House in Pittsburgh, PA. Materials and supplies to complete the blankets were provided by each agency. Employees served a shift at The Lighthouse Foundation Food Bank, Middlesex, PA on their distribution day. Employees gathered personal care items, household items, and clothing to donate to the Victim Outreach Intervention Center (VOICe) in Butler, PA. Employees gathered and sorted and delivered groceries for a Blessings in a Backpack program which was a partnership with Freeport High School to provide groceries during the summer to students who qualify for the free lunch program during the school year. These events are some of the ways we serve God by providing for the needs of His people. The following local organizations have received direct financial support from Concordia Community Support Services through sponsorships of events: Butler Catholic School, Butler County Community College, Butler Health System Foundation, Community Health Center, Concordia Lutheran Ministries Foundation, and Wesley Family Services. Total cost to the agency for sponsorships for fiscal year ended 6/30/2019 equated $3,673. CCSS made cash donations to various other 501(c)(3) organizations including: Concordia Lutheran Ministries Foundation Charitable Fund: $1,001, St Luke Lutheran Church: $4,000, Good Samaritan Endowment Fund: $200,000, as well as $665 in donations related to our Mission Events. Concordia Tele-CareGivers was supported with a total of $25,210, and Concordia Lutheran Ministries Parent: a total of $3,500,000. The donations are used to support other non-profits and further the mission of caring for God's people either through charitable care that supports patient's needs or to better serve both domestic and foreign communities through Christ-centered programs.
