The organization also partners with Concordia Lutheran Health and Human Care (EIN 25-0969458)to provide Medical Director services to the skilled nursing, outpatient rehabilitation, personal care, and hospice units on their Cabot, PA campus. Through the medical director services, the organization provides guidance and leadership to the operations of Concordia. The Medical Director previously served as a staff physician for the Allegheny Health Network and UPMC Physician Services. His work experience includes leadership roles in private practice as well as at both hospital systems and in hospice care, rehabilitation, and nursing home environments. The practice also includes two Nurse Practitioners, one specializing in Adult Gerontology Acute Care and one in Family Practice. Through the care administered at the locations guided by the medical directors the care services at Concordia achieve very low hospitalization, infection, and readmission rates. In addition to providing these services to the Concordia Lutheran Health and Human Care campus, recommendations that have allowed this consistent quality of care at that location have been implemented across the full Concordia Lutheran Ministries network, ensuring that all residents that entrust themselves to Concordia, will receive highest quality of care.
