Concordia Private Care has faithfully been offering top-quality non-medical services to people in the communities we serve. Many people who are ill or elderly need short or long-term assistance with normal day-to-day tasks. Our staff of personal caregivers undergoes continual training in the best ways to provide these types of services, customizing them to individual needs, with the patients independence, comfort, and safety in mind. Such tasks include but aren't limited to; assistance with bathing and dressing, ambulation (walking) and transfers, medication reminders, feeding, live-in companionship (24-hour care available).
Our Mission is to deliver quality home health services, in good faith, on behalf of those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion.
The mission of Concordia Tele CareGivers dba Concordia Private Care (CPC) is to deliver quality home care services, in good faith, on behalf of those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. Approximately 820 consumers were served across five counties in Western Pennsylvania. A total of 44,810 visits, equating to 61,405 hours of care, were delivered by our staff. We serve those over 65 in the community in order to help them age in place in their home as safely as possible. Our staff of caregivers helps their consumers with functions of everyday living including, but not limited to: bathing, grooming, toileting, light housekeeping, laundry, grocery shopping, and meal preparation. Our consumers are referred to us from a wide variety of sources. These individuals are often nursing home eligible for their level of care. Our staff of caregivers provides Personal Assistance Services to this group of consumers. Another source of consumer referral is the local county Area Agencies on Aging. We work with Allegheny, Butler, and Westmoreland county Options programs which fund personal care and home support services to financially qualified seniors. In addition, our organization accepts referrals from the VA to assist veterans who have been approved for in-home services including home support and personal care. Our administrative staff provides education to the community by speaking at different engagements on topics regarding senior independence. The CPC Private Duty Manager and the CPC Service Coordinators have addressed different groups of Butler County and Allegheny County seniors. The Private Duty Manager has attended several health fairs in order to provide information on CPC's available services including Medical Alert systems/Personal Emergency Response Services (PERS) and a Medication Management system. Our goal is to educate the community we serve, in the tools available to help them stay in their homes as safely as possible for as long as possible. We have continued to offer and arrange contracted spiritual visits for our consumers that were provided by Concordia Visiting Nurses at no added cost. On January 31, 2020, acting HHS Secretary Alex Azar first declared a public health emergency following the World Health Organization's recognition of the novel strain of coronavirus, COVID-19, as a pandemic. In response to this coronavirus outbreak, the state and federal governments have taken preventative or protective actions, such as imposing restrictions on travel and business operations and advising or requiring individuals to limit or forego their time outside of their homes. Like all home care entities, Concordia Private Care has faced their share of challenges during the COVID -19 Pandemic, but overall, the agency has seen a greater need for consumer companionship visits so there were no layoffs as a result of the pandemic. Family members have been unable to see their relatives for longer periods of time and so have taken the opportunity for private care staff members to meet their needs for companionship and even home support. Isolation concerns for family members have been alleviated by companionship provided, sometimes for short periods of time, but always in accordance with Concordia's mission and care for the people we serve. Daily high-level leadership meetings were established to assess and provide guidance to staff through every changing regulation surrounding the pandemic. Throughout the pandemic, education and guidance related to best practices and CDC protocol was provided by our corporate compliance team. This allowed our frontline workers to remain safe and to ensure that all visits were done in a way that would protect both the consumers and the caregivers from the spread of the virus. Employees were awarded a daily appreciation bonus for each shift worked. The agency worked with a corporate purchasing team to ensure appropriate levels of personal protective equipment (PPE), laboratory equipment, tests and other related COVID response items were purchased to provide care is the safest way possible. The agency purchased $1,637 of PPE. While many fundraisers and events in the area were canceled after the start of the pandemic, during the fiscal year CPC was able to support the following local organizations with direct financial support: BC3 Education Foundation, Inc., Butler Catholic School, Butler County Symphony Association, Butler Health System Foundation, Community Health Center, Freeport Football Boosters, Highlands Area Relay for Life, Knoch Track & Field Booster Association. Total donations and sponsorships made during fiscal year ended 06/30/2020 totaled $960. Employees of CPC were given the opportunity to participate in several mission events organized by the home and community-based agencies of Concordia. These are part of our ongoing Mission Committee that was formed to organize opportunities for employees to serve the community. In the summer of 2019, CTC employees partnered with Freeport High School to assist with their Blessings in a Backpack program. This program supplies the families of children on the free lunch program with groceries during the summer months when school is not in session. Our employees worked in teams which picked up groceries from Walmart and filled 20-25 bags with non-perishable items and dropped off the completed bags at the school. There were three food packing days that fell in the fiscal year ending June 30, 2020. A Servant Day/House Build was held on Sept 21, 2019 when employees partnered with St Luke Lutheran Church to construct the frame for a 1,500 square foot house. The team was assisted by the Center for Community Resources from Butler, PA and Crossroads Mission-Help Build Hope from Louisville, KY. Other service opportunities available that day were making fleece blankets for Ronald McDonald House, sewing dresses for girls in Tanzania, making greeting cards for Military families, preparing dry soup mix for local food pantries. For the Christmas 2019 season, employees received the Christmas wish lists of 24 children through a partnership with Butler County Children and Youth. Employees purchased gifts and wrapping paper to give to the families in the program. These events are some of the ways we serve God by providing for the needs of His people.
The Concordia Tele-Caregivers program is a free program available to any person in the Concordia service territory. Clients of the program are contacted via telephone each weekday morning by a volunteer just to ensure that the client is awake, functioning and able to answer their phone. During the phone conversation, we ensure that the client has taken steps to eat breakfast as well as take all directed medications. If the client does not answer their phone at their assigned time, action steps are taken to ensure that someone is reached who can either confirm the client's whereabouts or can go to the client's home to ensure that they are safe. This program provides additional comfort in one's home, and can give family members of elderly relatives living alone an additional sense of comfort. During the fiscal year ended 06/30/2020, 12 clients were provided with daily phone calls by a total of 3 volunteers. As part of our Concordia Cares Program, a multi-part program to lift our residents' and patients' spirits during the COVID-19 Pandemic, additional promotion of the Concordia Tele-Caregivers program occurred to be able to reach more clients interested in the service. We came to learn that several member churches were working to support elderly home-bound members in a similar fashion with daily or weekly touch points. We offered training and assistance with their growing programs considering our knowledge in the subject matter.
