Organizations Filed Purposes: The mission is to deliver quality home health services, in good faith and to minister to the unique needs of each individual with respect, dignity, and compassion. We serve with direct care services, community-based programs, disease management programs and spiritual care. We also support member 501(c)(3) organizations with distributed funds of $6,000,000. The following are programs that are not reimbursed to the agency. The Tele-Health program provides support to individuals in their home, at a non-reimbursed cost of $389,921 and we also provide spiritual care for those in need at a non-reimbursed cost of $86,129. For fiscal year ending 06/30/2019, a total of $61,859 in services was rendered as benevolent care.

Health care delivery and management: Provide community based programs, services, and facilities to seniors and disabled and ill persons in their homes. Includes nursing, therapy, home health aide, medical social work, and related services.

Concordia Visiting Nurses (CVN) program service accomplishments are driven by the core mission to provide excellent in-home health services to those who need them, in a faithful environment. CVN is part of a large continuum of care striving to succeed with an aging-in-place philosophy by providing a wide breadth of services in the home, preventing rehospitalization, enhancing home safety and monitoring chronic disease states. During the fiscal year ending June 30, 2019, approximately 15,178 patients were served across thirteen counties in Western Pennsylvania with an average daily census of 2,315. A total of approximately 296,378 visits (including 2,250 non billable visits associated with spiritual care, nutrition, etc.) were completed by various disciplines. Additionally, as part of the prevention efforts for the deterioration of existing health conditions, CVN provides Tele-Health monitoring free of charge to patients with chronic diseases like congestive heart failure. Tele-Health monitoring is a free service to the community but costs the agency $389,922.00 annually. The management team of Concordia Visiting Nurses works diligently to ensure that the agency operates at the highest level of quality. As part of that management role, the team continually evaluates its operations and makes changes in policy and practice to improve the agency's scope of practice. The organization is committed to providing the highest quality of care from nursing and therapy to specialized services such as IV therapy, wound care, palliative care, and mental health care, all from the comfort of the patient's home. CVN serves residents in the following counties; Allegheny, Armstrong, Beaver, Butler, Greene, Lawrence, Westmoreland, Washington and parts of Clarion, Fayette, Indiana, Mercer and Venango counties. CVN employs a team of Transitional Nurse Liaisons who serve the community by providing a variety of information, screenings, speaking topics on healthy living and disease prevention. They also team up with CVN nurses, social workers, and physical therapists to provide balance screening assessments, Health Chats and "Ask the Nurses" programs. Each month our Community Educator and our Transitional Nurse Liaisons schedule and attend several community gatherings to discuss our "Concordia Visiting Nurses Healthy Topic" of the month at health fairs, community speaker programs and events. Our goal is to reach as many people as possible in the community to promote wellness and teach prevention. Ideally, we would like to prevent the deterioration of existing health conditions and identify and teach early intervention for exacerbations of diseases and careful management to prevent hospitalizations. In providing screenings, we are attempting to be proactive in the early diagnosis and treatment of chronic disease conditions. Any senior adult with questionable screening results or symptoms are always referred to their physician for further testing and treatment and timely follow-up occurs with the senior by a member of our team. We are receptive to and always looking for ways to enhance Concordia's exposure throughout the community. These free education and screening events are conducted at a variety of apartments, housing centers, senior centers, events at local malls, churches, civic centers, and other health care facilities. 673 flu shots were provided by CVN to employees and to the community at several Flu Clinics which were held at various sites across our service area during the 2018-2019 fiscal year. Other community education that was provided was an event at Rural Valley Senior Center (Rural Valley, PA) to educate on Aging & Memory and Compensatory Strategies. This was presented by the Therapy Manager and Marketing Liaison, who were also able to renew CVN services for patients and caregivers at the facility while there. A CVN Social Worker and a CVN Telehealth Installer held a Focus-on-Health-and-Safety education event that included telehealth demonstrations, blood pressure screenings and health checks. The Therapy Manager presented a talk on Exercise-As-We-Age at the Armstrong County Elderton Senior Center. This included basic principals to start an exercise program and progress as able, and suggested modifications given the varied health status of those attending, complete with some participation in various exercises. A CVN Physical Therapy Assistant participated in a Multiple Sclerosis (MS) Support Group that was held at the Puckety Presbyterian Church in Lower Burrell, PA. The event included a talk on exercising at various stages of the progression of MS, a demonstration, and group participation. During the 2018-2019 fiscal year, the following (but not limited to) organizations have received direct financial support from CVN through donations and sponsorships of various events: ACMH Foundation, Alzheimer's Association, AMD3 Foundation, Armstrong County Healthcare Consortium, Beaver County Chamber of Commerce, Butler AM Rotary Club, Butler County Healthcare Consortium, Butler County Sports Hall of Fame, Butler Health System Foundation, Concordia Lutheran Ministries Foundation, Heritage Valley Beaver Auxiliary, Heritage Valley Beaver Foundation, Knoch Performing Arts Boosters, Lutheran Service Society, Middlesex Township Volunteer Fire Company, Montour Golf Boosters, St Clair Hospital, St Luke Lutheran Church, SWPA Area Agency on Aging, Tom Desiderio Memorial Fund, Washington Health System Foundation. Total donations and sponsorships made during fiscal year ended 6/30/19, equated $46,046. Additional funding via distributions to 501(c)(3) member hospital systems at a total of $6,000,000 have expanded like-minded mission within the geographical communities we serve. Many employees of CVN volunteer their time to serve at the Annual Summer Festival and Octoberfest, both of which are sponsored by Concordia Lutheran Health and Human Care and are open to the public as a way of providing entertainment, food and refreshments and a feeling of community. One CVN employee in particular devotes an extra 40 hours per year to coordinate the Kids Area for each festival which includes activities, crafts and face painting, all of which are provided free to the public. We also have an employee who volunteers to take an annual trip to an orphanage in Guatemala to deliver medical supplies, personal care items, clothing, and toys to those in need. Some of the supplies are medical items donated by CVN and others were gathered and donated by fellow CVN employees. Employees of CVN were given the opportunity to participate in several mission events organized by the parent company Concordia Community Support Services. These are part of our ongoing Mission Committee that was formed to organize opportunities for employees to serve the community. The events completed this year include: A Servant Event at Prince of Peace in Farrel, PA where employees were able to serve food for many families as well as make snow cones, bag popcorn, run a bounce house and help with the clean-up at the block party for community children. St Luke Lutheran Church House Build where employees of CVN and members of the St Luke congregation built the framing for a dormitory for teens who need a place to live post-foster care age This event took place at the church in Cabot, PA where there were also opportunities to serve by making dresses and blankets and marking verses in Bibles for the Allegheny Co Jail. Give Thanks in a Tote event where thanksgiving meal supplies, a gift card, and an encouraging devotion were placed in a tote and given to nearly 200 patients under our care. Employees volunteered their time to serve meals to 120 individuals at the North Side Homeless Alliance Winter Shelter in Pittsburgh, PA and sort through donations of winter clothes. CVN employees participated in a Fleece Blanket Mission Event where employees volunteered during their lunch break to cut and tie fleece blankets that were donated to families at the Ronald McDonald House in Pittsburgh, PA. Materials and supplies to complete the blankets were provided by CVN. Serve a shift at The Lighthouse Foundation Food Bank, Middlesex, PA on their distribution day. Employees gathered personal care items, household items, and clothing to donate to the Victim Outreach Intervention Center (VOICe) in Butler, PA. Blessings in a Backpack which was a partnership with Freeport High School to provide groceries during the summer to students who qualify for the free lunch program during the school year. These events are some of the ways we serve God by providing for the needs of His people.
