ELWYN CALIFORNIA WAS DEVELOPED SPECIFICALLY TO PROVIDE SUPPORT TO CHALLENGED INDIVIDUALS MOVING INTO THE COMMUNITY FROM STATE INSTITUTIONAL SETTINGS. ELWYN CALIFORNIA ALSO PROVIDES SERVICES TO INDIVIDUALS CURRENTLY LIVING IN THE (CONTINUED ON SCHEDULE O)
The provision of Community Day and residential programs and social rehabilitation services to disabled persons previously residing in state (Continued on Schedule O)
Responsible for the transfer of developmentally disabled persons from state institutions to community group homes. Individualized habilitation and behavior support plans for these clients were generated and implemented. Community living arrangements were designed and opened for 186 special-needs individuals residing in community group homes. These individuals are being integrated into the community in order to enjoy active, productive lives.
Work opportunities, supported employment and adult day care programs serving approximately 279 individuals are operated to provide employment and social integration opportunities to individuals with intellectual disabilities.
