Organizations Filed Purposes:
To respond to the physical, social and spiritual needs of the Hispanic Community with a focus on Spanish speaking high risk pregnant women and sick or disabled babies and children. Our goal is to help them find appropriate health care to meet the physical needs and to minister to their spiritual needs.
We provided transportation, translation, insurance elgibility counseling, and care management services to over 200 families, attending over 20 medical appointments per month, and driving in excess of 5,000 miles. We took 85 family members to a week end retreat to focus on Spanish based spirtual instruction, and provided respite care for families of very high need disabled children. We held a weekly session for the moms of the families we minister to, providing day care for their speical need children, while the mothers focused on sharing best practices, coping skills, and received Bible Study. We averaged 2 home visits each week. In addition, we communicate with family members by text to coordinate patient care, or to manage care. We coordinate medical appointments, help parents understand treatment protocols, and reinforce Doctor's orders to assure care consistent with prescribed medical treatment plans. We communicate with school nurses to assure continuity of care that would not otherwise exisit because of language barriers. We provided emotional support to parents of medically fragile children, and to high risk pregnant women.
