PROVIDE SERVICES TO INDIVIDUALS WHO HAVE INTELLECTUAL DEVELOPMENTAL DISABILITIES, DRUG AND ALCOHOL PROBLEMS, AND BEHAVIORAL HEALTH ISSUES.
MERAKEY DELAWARE COUNTY HAS 7 ADULT BEHAVIORAL HEALTH RESIDENTIAL PROGRAMS SUPPORTING 68 INDIVIDUALS. THE PROGRAMS OFFER SEVERAL LEVELS OF CARE. THE GOAL OF THE PROGRAM IS TO HELP RESIDENTS BECOME AS INDEPENDENT AS POSSIBLE, EMPHASIZING RECOVERY. THE MOST RESTRICTIVE PROGRAM IS THE LONG TERM STRUCTURED RESIDENCE (LTSR), THREE COMMUNITY REHABILITATION RESIDENCES (CRR) WITH TWO MAXIMUM CARE / 24-HOUR SUPERVISION AND ONE MODERATE CARE THAT PROVIDES LESS STAFF SUPPORT TO INDIVIDUALS IN SERVICE WHO ARE CLOSE TO MEETING THEIR GOALS. THE OTHER THREE MERAKEY DELAWARE COUNTY RESIDENTIAL PROGRAMS ARE ASSISTED LIVING SITES. MERAKEY DELAWARE COUNTY HAS HELPED MANY RESIDENTS MOVE THROUGH THE LEVELS OF CARE TO ULTIMATELY LIVE INDEPENDENTLY WITH MINIMAL SUPPORTS.THE MERAKEY DELAWARE COUNTY BLENDED CASE MANAGEMENT (BCM) PROGRAM SERVICES OVER OF 800 INDIVIDUALS. THE BCM PROGRAM IS DESIGNED TO SERVE ADULTS AND CHILDREN IN THE COMMUNITY WHO HAVE A MENTAL HEALTH DIAGNOSIS. USING AN INDIVIDUALIZED APPROACH TO SERVICE DELIVERY, MERAKEY DELAWARE COUNTY STRIVES TO PROMOTE COMMUNITY INTEGRATION AND INDEPENDENCE. THE PROGRAM OFFERS LINKAGE AND REFERRAL TO COMMUNITY RESOURCES, REHABILITATION, CRISIS AND SUPPORT SERVICES. THE SERVICES PROVIDE A UNIQUE MIX OF INTENSIVE CASE MANAGEMENT (ICM) AND RESOURCE COORDINATION (RC) PROGRAMS. PARTICIPANTS CAN FLUCTUATE BETWEEN ICM AND RC LEVEL OF CARE WITHOUT SWITCHING CASE MANAGERS OR PROVIDERS. IN ADDITION, BCM OFFERS NURSE NAVIGATION SERVICES TO ASSIST AND SUPPORT PARTICIPANTS HELPING WITH MEDICAL NEEDS AND TO BRIDGE THE GAP BETWEEN PRIMARY CARE DOCTORS, PSYCHIATRIST AND COMMUNITY RESOURCES. PARTICIPANTS ALSO HAVE AN ADDED BENEFIT OF BEING LINKED TO A CERTIFIED PEER SPECIALIST THAT ASSISTS WITH THE DEVELOPMENT OF COMMUNITY ROLES, INDIVIDUAL ADVOCACY, SELF-HELP, SELF-IMPROVEMENT AND SOCIAL NETWORKING.DURING FY20, MERAKEY DELAWARE COUNTY HAD THE FOLLOWING PROGRAM SERVICE ACCOMPLISHMENT HIGHLIGHTS:- INTEGRATION OF CARE PROVIDED HERE, INCLUCING PRIMARY CARE DOCTORS, PSYCHIATRIST AND COMMUNITY RESOURCES. PARTICIPANTS ALSO HAVE AN ADDED BENEFIT OF BEING LINKED TO A CERTIFIED PEER SPECIALIST THAT ASSISTS WITH THE DEVELOPMENT OF COMMUNITY ROLES, INDIVIDUAL ADVOCACY, SELF-HELP, SELF-IMPROVEMENT AND SOCIAL NETWORKING. THEY ALSO HAVE NURSE NAVIGATORS TO SUPPORT PHYSICAL HEALTH NEEDS OF INDIVIDUALS, WHICH SUPPORTS THE GOAL OF INTEGRATED HEALTH. IN ADDITION, THE PEER SUPPORT PROGRAM OFFERS WHOLE HEALTH ACTION MANAGEMENT (WHAM) WHICH RESULTED IN ITS FIRST GRADUATION THIS YEAR. - MERAKEY DELAWARE COUNTY WAS AWARDED A FEDERAL SAMHSA GRANT (FOR TWO YEARS (5/1/2020-4/30/22) FOR CONTINUED SUPPORT OF THE CERTIFIED COMMUNITY BEHAVIORAL HEALTH CLINIC (CCBHC). - MERAKEY IDD WAS AWARDED A GRANT IN THE AMOUNT OF $8,500 TO PROVIDE THERAPEUTIC EQUIPMENT AND ACTIVITIES TO OUR INDIVIDUALS.
