CHI St. Joseph Children's Health improves the health of children in need within Lancaster County, Pennsylvania.
CHI St. Joseph Children's Health (SJCH) was originally incorporated in 1985. It is a 501(c)(3) tax-exempt organization whose purpose, since July 2000, has been to improve the health of children and families in the Lancaster, PA community. SJCH has provided community-based health care services, support and education and has collaborated with other organizations to identify and address unmet health needs of children and families in the community. It has advocated for systemic change with specific concern for children and families who are economically disadvantaged while also promoting an understanding and commitment to the concept of healthy communities. Since 2004, SJCH has focused its services to children from birth to high school graduation. The members of the board of directors of SJCH, the majority of whom are independent community members, have supported the goals and programs of the organization by setting the vision and actively participating in the strategic planning process. SJCH services included the provision of children's health education programs, children's health advocacy and children's behavioral health/psychiatric services (including family wellness coaching and case management services) to improve the health of children and families in need. The behavioral health services provided by SJCH primarily serve children who are on medical assistance, chip or who are uninsured, though all children regardless of insurance status or ability to pay are welcome. Community service has always been at the core of SJCH activities. Each year services and programs are designed and expanded to promote a healthy community. The programs and services that are described throughout this report not only serve the community, but also reduce the burden on the government. For example, if SJCH did not provide charity care, the burden of providing charity care would fall on the two federally qualified health centers and the multiple emergency rooms operating in Lancaster county. SJCH is included in the official catholic directory as a tax-exempt health care organization. Community Benefit Approach Since July 2004, CHI St. Joseph Children's Health has been intimately involved in community health collaboration. SJCH continues its deep commitment and involvement in the community health collaborative and planning today including playing a leading role in the local public health collaborative, mental well-being collaborative and various social service collaboratives throughout the community. CHI St. Joseph Children's Health's community benefit plan is based on our understanding of unique community needs that were derived from a collaborative needs assessment, focus groups and a survey conducted with community partners. In February 2014, the board of directors adopted a new strategic plan which expanded the scope of services provided by SJCH in the organization's efforts to improve the health of children in the Lancaster community. While committed to the provision of high-quality clinical services, the plan recognized that efforts must be undertaken to address the disproportionate number of children in the community identified as uninsured. State-wide approximately 5% of children in the commonwealth are uninsured. In Lancaster county, data indicated almost 16% of children were without even basic health coverage. While also assisting families in understanding and effectively utilizing health benefits and programs. Finally, the plan called for the board to annually scan the environment for additional health needs of children and families in the community upon which SJCH could have a positive impact either directly or in partnership with other providers in the community. This on-going community scan resulted in the creation of SJCH's behavioral health and psychiatric services which launched in the summer of 2017 and the healthy community transformation initiative focused on improving the health outcomes of Columbia Borough. Uncompensated Care SJCH provides behavioral health services to economically disadvantaged children, those who are on Medicaid and CHIP and those who are not covered by insurance. Uninsured families are eligible to participate in the St. Joseph access program - a sliding fee discount program which offers discounts of 100% to families reporting incomes at or below 200% of the federal poverty level (FPL). Additional discounts are provided on a graduated scale for families earning between 205% and 300% of FPL. SJCH is able to submit claims for reimbursement to Medicaid and CHIP plans as well as commercial insurers. Community Outreach for The Poor Advocacy/Case Management CHI St. Joseph Children's Health launched a children's health advocacy/case management program in fiscal year 2015. The program works with children and families to assist families in securing and accessing appropriate health benefits and services to assure that every child in the Lancaster community has the opportunity to grow up happy and healthy. The program focuses upon families who are uninsured or under-insured and supports families. Any family struggling in accessing health services and benefits may access the program. Healthy Communities Initiatives In 2017, SJCH transitioned its grant-making program with the goal of increasing the impact of dollars spent. The Healthy Columbia Project was created and funds traditionally awarded as small grants to local organizations were redirected to services and investments in the Borough of Columbia, PA with the goal of transforming the health outcomes of the local community and improving the health of children and families residing in the Borough. The Borough has a median income significantly less than the rest of Lancaster County and is lacking in essential social services including child care and health navigation services which are now being planned or provided by SJCH. The program focuses on the creation of safe communities and neighborhood, health and wellness programming and access, early childhood experience and education and fresh food access and nutrition. Children's Health Education SJCH provides classroom and community children's health education. While the program is expanding to include other health needs, fiscal year 2016 efforts focused upon oral health education, early childhood health, wellness and hygiene and behavioral health. Programming was provided to children from preschool through grade 6 as well as the creation of professional development sessions for early childhood and elementary school teachers. In total, over 18,000 children were taught through the classroom and community education sessions.
