NEPA COMMUNITY HEALTH CARE IS COMMITTED TO PROVIDING QUALITY HEALTH CARE TO UNDERSERVED RURAL COMMUNITIES IN SUSQUEHANNA COUNTY, PENNSYLVANIA AND SURROUNDING AREAS. THE POPULATION IS APPROXIMATELY 44,000, WITH OVER 13% LIVING IN POVERTY, INCLUDING 20% OF THOSE UNDER THE AGE OF 19, AND 22.8% OF SENIOR CITIZENS. IN ORDER TO BETTER SERVE THIS POPULATION, WE PROVIDE PRIMARY CARE AND INTEGRATED BEHAVIORAL HEALTH AT THREE OFFICES LOCATED STRATEGICALLY TO IMPROVE ACCESS TO CARE FOR THOSE WITH TRANSPORTATION CHALLENGES IN THIS RURAL REGION. WE OFFER A SLIDING FEE DISCOUNT FOR THOSE WITHOUT THE RESOURCES TO PAY FOR THEIR CARE, ALONG WITH THE 340B DRUG PROGRAM WHICH GREATLY REDUCES THE COST OF MEDICATION FOR THE INDIGENT. BOTH EARLY AND LATE HOURS ARE OFFERED AT ALL LOCATIONS WITH CONSIDERATION OF PATIENTS' WORK AND SCHOOL SCHEDULES. WE ARE THE ONLY PRENATAL CARE PROVIDER IN THE COUNTY AND HAVE WIC CLINICS WITHIN OUR BUSIEST HEALTH CENTER. WOMEN AND THEIR FAMILIES CAN BE ASSURED OF THE FULL RANGE OF PHYSICAL AND MENTAL HEALTH CARE AND CONNECTION TO SUPPORT SERVICES AT OUR SITES. MEDICATION ASSISTED TREATMENT FOR SUBSTANCE ABUSE DISORDERS IS ANOTHER SERVICE THAT WE PROVIDE IN AN INTEGRATED MODEL TO ELIMINATE THE STIGMA OF ACCESSING MENTAL HEALTH AND RECOVERY SERVICES WHILE PROVIDING COMPREHENSIVE PRIMARY CARE. OUR PATIENT POPULATION INCREASED TO OVER 5,000, WITH EXPANSIONS IN OUR INTEGRATED MAT AND BEHAVIORAL HEALTH SERVICES. THE COVID19 PANDEMIC HAS FORCED CHANGES IN PROVISION OF CARE WHICH ULTIMATELY IMPROVED ACCESS IN MANY CASES. ADDITION OF TELEHEALTH FOR ALL SERVICE LINES ENABLED PATIENTS TO ACCESS NON-URGENT CARE FROM THE SAFETY OF THEIR HOMES, WHICH IS ESPECIALLY ADVANTAGEOUS TO THE FRAIL ELDERLY REQUIRING CHRONIC DISEASE MANAGEMENT, AS WELL AS THOSE WITH TRANSPORTATION CHALLENGES. WE HAVE ESTABLISHED AND EXPANDED OUR CARE MANAGEMENT SERVICE LINE, ADDRESSING THE SOCIAL DETERMINANTS OF HEALTH WHICH DECREASE ACCESS TO CARE, AND CLOSING THE LOOP ON GAPS IN CARE TO IMPROVE HEALTH OUTCOMES.
