Non-profit regional network providing a continuum of living and care options for older adults.In addition to Westminster Place, Woodside Place, and Southminster Place, three personal care facilities it owns, Presbyterian SeniorCare is the sole member of numerous nonprofit membership corporations that provide a regional system of housing, personal care, Alzheimer's and dementia care, long-term and rehabilitation healthcare, and community services for older adults. Of note are its 28 affordable housing communities serving persons with disabilities and low-income seniors through a service-enriched model that supports aging in place and minimizes the need for institutional care. Presbyterian SeniorCare gives back to its local communities and provides charitable care to the seniors it serves who are no longer able to pay for their care. In 2019 across its regional network, it provided $574,200 in community support and a combined total of $26.5 million in benevolent and uncompensated care.

For older adults who are not able to live alone but don't need nursing home care, we offer personal care in three facilities - Westminster Place, Woodside Place and Southminster Place - bringing together modern residential living options with caring and professional staff who provide assistance with daily tasks and create opportunities for socialization. We served approximately 300 elderly residents in our three personal care facilities, subsidizing about $1,480,900 of care for those individuals who could no longer afford to pay.These personal care facilities are part of the Presbyterian SeniorCare Network which serves more than 6,500 older adults annually through 56 communities/service lines across 10 western Pennsylvania counties. Presbyterian SeniorCare has been instrumental in developing, and is the sole member of, numerous not-for-profit membership corporations that provide older adults with living and service options including: a regional system of housing for low income seniors and persons with disabilities, personal care, Alzheimer's and dementia care, long-term and rehabilitation healthcare, and home health, and community services for older adults. Additionally, in collaboration with Jewish Association on Aging and UPMC, we are a sponsor of Community LIFE, a non-profit program that provides comprehensive health and social services to approximately 600 frail, elderly adults so that they can continue to live in their homes. Presbyterian SeniorCare strives to participate with the provider networks of all of the major insurers and health systems in the markets we serve.The Internal Revenue Service has determined that Presbyterian SeniorCare is exempt from federal income taxation under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. However, as a responsible corporate citizen, Presbyterian SeniorCare gives back to its local communities through promotion of health and wellness efforts, education and training programs, financial and in-kind donations, and special projects. Guided by its faith-based not-for-profit mission, Presbyterian SeniorCare provides $2.3 million in benevolent care annually to residents in our personal care communities who no longer have the means to pay. In 2019, across its regional network, it provided $574,200 in community support and a combined total of $26.5 million in benevolent and uncompensated care.Based upon the development of Woodside Place in 1991, Presbyterian SeniorCare created the Woodside Program, a model of Alzheimer's and dementia care embraced by others, including the skilled nursing communities and Adult Day programs in our regional network. Today Presbyterian SeniorCare is recognized as a Dementia Care Center of Excellence and is partnering in the training of hospital emergency department staff and first responders.The Woodside model inspired the development of Woodside Place of Erie, which opened in fall 2015 at our Manchester Commons campus. In 2017, construction began for Woodside Place of Washington, the first free-standing, residential dementia care community in Washington County, which is scheduled to open in Spring 2020.Recognizing the pervasiveness of cognitive impairment among older adults, all employees since 2015, including even those who were not direct caregivers, receive training through the National Alzheimer's Association. Additionally, direct care givers complete an immersion experience that is over and above the training provided to all employees. Additionally, Presbyterian SeniorCare has developed Dementia360, which begins a phased rollout in 2019. Dementia360 extends dementia care management into the home setting to support family caregivers who are caring for persons with Alzheimer's disease and related dementias. The program aims to prevent unnecessary hospitalizations and delay nursing home placement while enhancing the quality of life for both the person living with the disease and the caregiver. A generous grant from a local foundation enables Dementia360 care management services to be offered at no cost to clients up through December 2020.In keeping with our person-centered care philosophy, our 7 skilled nursing communities have been transformed from the traditional institutional setting and medical model of yesteryear to home-like environments that deliver personalized care with an emphasis on enhancing quality of life. More than 50% of persons served annually are covered by Medical Assistance. As a Center of Rehab Excellence, Presbyterian SeniorCare developed the MyLife Program, a holistic approach to recovery, which includes the physical environment, the delivery of hospitality and personalized clinical care provided by an expert team that gets patients back home and back to their life.We support older adults wherever they are on life's journey- from low to moderate income supportive housing to programs for those who wish to stay at home. Presbyterian SeniorCare is the sponsor of 28 supportive and affordable housing communities serving persons with disabilities and low-income seniors through a service-enriched model that supports aging in place and delays/prevents the need for institutional care. Additionally, through its management affiliate, SeniorCare Network, it provides management services to 9 low-income housing communities and a program serving 10 counties in western Pennsylvania called SeniorCare Management Assistance (SCMAF), which provides housing assistance and related supportive services to help homeless or at-risk low-income persons living with HIV/AIDS and their families achieve housing stability. In 2016, its affiliate, Presbyterian SeniorCare at Home, launched a Home Health Agency to further enhance choices for care.Its affiliate, Longwood at Home, is western Pennsylvania's only licensed Continuing Care at Home program. The program is a way for people who are healthy today to maintain their current lifestyle but also put a plan in place for the future so quality care and support will be there if and when they need it.In 2002, Presbyterian SeniorCare began a deep collaboration with Family Hospice and Palliative Care to develop Woodwell, a comprehensive program of care and supportive services for older adults facing life-limiting illnesses who reside in our communities. In 2007, the Woodwell Program received the national 2007 American Hospital Association Circle of Life Award as an innovative model of hospice care within a long-term care setting. In 2015, a formal partnership was established between Family Hospice, Presbyterian SeniorCare and UPMC to enable high quality hospice services to reach the broader western Pennsylvania community.The footprint of Presbyterian SeniorCare expanded when the Commonwealth of Pennsylvania Attorney General's Office reviewed and had no objection to the affiliation of Presbyterian SeniorCare and Presbyterian Homes of Lake Erie in March 2015, and the affiliation with Shenango Presbyterian SeniorCare in New Wilmington, PA in January 2016. Since its inception in 1928, Presbyterian SeniorCare has been committed to service excellence and Making Aging Easier for older adults and their families. Built upon the principles of "servant leadership," we are dedicated to investing in our people and promoting a standard of care and personalization that focuses on the highest quality of care possible. In 2016, the Presbyterian SeniorCare Network was among the first aging services organizations in the U.S. to have achieved CARF accreditation in six different areas. In addition to being awarded a Five-Year Term of Accreditation for meeting the "Network" standards for senior services and living options, the Presbyterian SeniorCare Network also achieved accreditation in five additional program areas certified by CARF: Continuing Care Retirement Communities (CCRCs), Assisted Living, Person-Centered Long-Term Care Communities (PCLTCC), Home and Community Services, and Dementia Care Specialty Program.Beginning in 2013, a deliberate multi-year, multi-phase education and awareness effort was launched to foster diversity, equity and inclusion so our Network will be ready to serve an increasingly diverse aging population and to appropriately engage and support an increasingly diverse workforce. Based upon a generous local foundation grant, Presbyterian SeniorCare developed a career pathways program in collaboration with middle and high schools to create awareness of healthcare careers in senior services and long-term care. In 2018, two programs were piloted at the Oakmont and Washington campuses: The Youth Engagement Pathways (YEP) summer volunteer program for teens 13 to 16 and the High School Career Pathways program, a curriculum-based initiative in partnership with three Pittsburgh Public Schools (Carrick, Perry and Westinghouse) and in Washington County, Trinity High School and Western Area Career and Technology Center (WACTC).
