Organizations Filed Purposes: THE LCD'S MISSION IS TO DELIVER FREE, QUALITY LEGAL SERVICES TO LOW INCOME PEOPLE WITH PHYSICAL AND MENTAL HEALTH DISABILITIES AND THE DEAF OR HARD OF HEARING COMMUNITY TO HELP THEM OVERCOME LEGAL OBSTACLES THAT WOULD OTHERWISE AFFECT THEIR INDEPENDENCE, HEALTH, SAFETY, OR QUALITY OF LIFE.

IN 2008 LCD MADE A STRATEGIC DECISION TO MOVE AWAY FROM TELEPHONE INTAKE AS ITS SOLE MEANS OF OFFERING LEGAL SERVICES TO PEOPLE WITH PHYSICAL DISABILITIES IN THE PHILADELPHIA REGION. SINCE 2008 LCD HAS DEVELOPED NUMEROUS PARTNERSHIPS IN THE COMMUNITY THAT HAVE BROADENED ITS ABILITY TO REACH MORE PEOPLE WITH DISABILITIES IN NEED OF LEGAL SERVICES. LCD LOOKS HOLISTICALLY AT A CLIENT'S LIFE AND WORKS WITH OTHER AGENCIES TO CONTINUE TO PROVIDE INDEPENDENCE, ACCESS TO THE COURTS AND COMMUNITY, IMPROVED HEALTHCARE OUTCOMES AND A BETTER QUALITY OF LIFE FOR THOSE WITH DISABILITIES. "GOING TO THE CLIENT" IS AN EXCITING, INNOVATIVE APPROACH THAT ENABLES LCD ATTORNEYS TO PROVIDE LEGAL ADVOCACY TO COMPLEMENT THE TREATMENT PROVIDED BY MEDICAL PROFESSIONALS, THERAPISTS, SOCIAL WORKERS, AND OTHER INDIVIDUALS WHO PROVIDE SERVICES TO PEOPLE WITH DISABILITIES. LCD'S PROGRAMS INCLUDE: COMMUNITY LEGAL OUTREACH CLINICS (CLOCS) IN PARTNERSHIP WITH PRO-BONO VOLUNTEERS FROM AREA LAW FIRMS LCD CONDUCTS CLOCS WITH COMMUNITY BASED ORGANIZATIONS THAT SERVE INDIVIDUALS WITH DISABILITIES IN THE PHILADELPHIA AREA. IN 2019 LCD AND ITS PRO BONO PARTNERS PROVIDED CLOCS AT ASSOCIATED SERVICES FOR THE BLIND, INGLIS HOUSE (A RESIDENTIAL LIVING FACILITY FOR PEOPLE WITH DISABILITIES IN NEED OF 24 HOURS ASSISTED CARE), 4 FREEDOMS APARTMENTS (A SENIOR LIVING COMMUNITY IN GERMANTOWN) AND FACE TO FACE GERMANTOWN (A NOT-FOR-PROFIT 501(C)(3) HUMAN SERVICES ORGANIZATION DEDICATED TO THE HEALTH, WELL-BEING, AND STABILITY OF THE COMMUNITY) TO PROVIDE FREE LEGAL SERVICES FOR PEOPLE NEEDING PLANNING DOCUMENTS AND CREDIT SCORE REPAIR. MEDICAL LEGAL PARTNERSHIPS (MLP) MLP IS A HEALTHCARE DELIVERY MODEL THAT INTEGRATES LEGAL SERVICES INTO A HEALTHCARE SETTING. THROUGH AN INTERDISCIPLINARY, HOLISTIC TEAM APPROACH, ATTORNEYS AND HEALTH CARE PERSONNEL COMBINE LAW AND MEDICINE TO REMOVE LEGAL BARRIERS THAT PREVENT ACCESS TO HEALTHCARE AND MITIGATE SOCIAL DETERMINANTS THAT PREVENT POSITIVE HEALTH CARE OUTCOMES. CURRENTLY, LCD PARTNERS WITH THE FOLLOWING HEALTH CARE PROVIDERS: MAGEE REHABILITATION HOSPITAL; FAMILY PRACTICE & COUNSELING NETWORK (FPCN) - THREE FEDERALLY QUALIFIED NURSE MANAGED HEALTH CENTERS; PHILAKIDS MLP AT ST. CHRISTOPHER'S HOSPITAL FOR CHILDREN; PROJECT HOME'S STEPHEN KLEIN WELLNESS CENTER IN NORTH PHILADELPHIA; THE HUB OF HOPE - PROJECT HOME'S NEW DROP-IN CENTER IN SUBURBAN STATION, PHILADELPHIA THAT OFFERS A SAFE HAVEN AND SERVICES FOR INDIVIDUALS EXPERIENCING CHRONIC HOMELESSNESS. EDUCATIONAL OUTREACH LCD ALSO PROVIDES EDUCATIONAL OUTREACH TO LAWYERS, MEDICAL PROFESSIONALS, SOCIAL SERVICE AGENCIES AND SCHOOLS TO EDUCATE THEM ABOUT LEGAL ISSUES FACING PEOPLE WITH DISABILITIES AND ABOUT THE AVAILABILITY OF OUR SERVICES. LCD PROGRAM ACCOMPLISHMENTS IN 2019 LCD PROVIDED LEGAL SERVICES TO MORE THAN 1,600 PEOPLE. THE MAJORITY OF THESE CASES CONSISTED OF PEOPLE SEEKING LEGAL SERVICES FOR FAMILY LAW ISSUES, PUBLIC BENEFITS, HOUSING AND EVICTION ISSUES, PLANNING DOCUMENTS, AND CONSUMER PROTECTION. THE MAJORITY OF THE CASES CAME OUT OF PHILADELPHIA COUNTY. LCD CONTINUES TO EXPAND ITS PROGRAMS TO ADDRESS THE LEGAL NEEDS OF ITS CLIENT POPULATION. IN 2019 LCD HAD STAFF ATTORNEYS ONSITE AT ALL OF ITS MLP LOCATIONS INCLUDING PHILAKIDS MLP AT ST. CHRISTOPHER'S HOSPITAL FOR CHILDREN; THE FAMILY PRACTICE AND COUNSELING NETWORK (FPCN), A NETWORK OF THREE FEDERALLY QUALIFIED NURSE MANAGED HEALTH CENTERS (FQHC) IN PHILADELPHIA: HEALTH ANNEX IN SOUTHWEST PHILADELPHIA, ABBOTSFORD FALLS FAMILY PRACTICE AND COUNSELING IN GERMANTOWN AND THE ELEVENTH STREET FAMILY HEALTH SERVICES OF DREXEL UNIVERSITY IN NORTH PHILADELPHIA; THE STEPHEN KLEIN WELLNESS CENTER, PROJECT HOMES HEALTH CENTER IN NORTH PHILADELPHIA; THE HUB OF HOPE, PROJECT HOMES WALK IN CENTER THAT PROVIDES SERVICES TO PEOPLE EXPERIENCING CHRONIC HOMELESSNESS; AND MAGEE REHABILITATION HOSPITAL. THE ONSITE PRESENCE ALLOWS THE ATTORNEYS TO PROVIDE IMMEDIATE ADVICE, ADVOCACY, OR IN-COURT REPRESENTATION UPSTREAM BEFORE LEGAL ISSUES REACH A CRISIS POINT. LIKEWISE, IT ALLOWS THE ATTORNEYS TO CONDUCT TRAININGS AND ONE-ON-ONE CONSULTATIONS OVER THE COURSE OF THE YEAR. THE PURPOSE OF THE TRAININGS ARE TO: (1) RAISE AWARENESS ABOUT THE LEGAL SERVICES AVAILABLE ONSITE FOR PATIENTS/CLIENTS IN NEED AND (2) EDUCATE THE HEALTHCARE PROVIDERS AND/OR SOCIAL SERVICE PROFESSIONALS ABOUT LEGAL ISSUES PREVALENT AMONG THEIR POPULATIONS AND HOW TO MAKE APPROPRIATE REFERRALS FOR LEGAL SERVICES. THE MLP MODEL OF CARE AND THE LCD ONSITE LEGAL SERVICES WORK TO COLLABORATIVELY IDENTIFY AND ADDRESS LEGAL ISSUES PROACTIVELY IN AN EFFORT TO IMPROVE THE HEALTH AND QUALITY OF LIFE FOR THE INDIVIDUALS THEY SERVE. LEGAL TRAININGS ARE PROVIDED TO KEY PARTNERS IN EACH VENUE, E.G. PHYSICIANS, PEDIATRIC RESIDENTS, CASE MANAGERS/SOCIAL WORKERS, AND NURSE PRACTITIONERS. THE LCD ATTORNEYS EDUCATE CLINICAL PARTNERS ABOUT THE MLP MODEL AND EXPANDING PATIENT ASSESSMENTS TO IDENTIFY SOCIAL DETERMINANTS IMPACTING THEIR HEALTH FOR WHICH THERE ARE LEGAL REMEDIES (E.G. UNSAFE HOUSING, FOOD INSECURITY, LACK OF UTILITIES, DOMESTIC VIOLENCE, FAMILY ISSUES, FOOD AND UTILITY INSECURITY). THE CONSULTATIONS BETWEEN LCD ATTORNEYS AND HEALTHCARE PROVIDERS AT EACH OF THE SITES HAVE INCREASED IN NUMBER AS THE PARTNERSHIPS HAVE EXPANDED AND MATURED. IN 2019, SEVENTY-SEVEN ATTORNEYS FROM BLANK ROME, DUANE MORRIS, BALLARD SPAHR, REED SMITH, DECHERT, COZEN O'CONNOR, DRINKER, DILWORTH PAXSON, COMCAST AND SOLE PRACTITIONERS HAVE RECEIVED TRAINING AND TAKE PRO BONO REFERRALS FROM LCD FOR GUARDIANSHIP CASES COMING OUT OF ST. CHRISTOPHER'S AND CHILDREN'S HOSPITAL OF PHILADELPHIA AS WELL AS PLANNING DOCUMENTS, CREDIT REPAIR AND EXPUNGEMENTS. IN 2019 LCD RECEIVED THE 2019 COMCAST NBCUNIVERSAL PROJECT INNOVATION AWARD FOR ITS MLP INITIATIVE, RECOGNIZING THE INNOVATIVE, COLLABORATIVE NATURE OF THIS SERVICE MODEL. LCD ALSO RECEIVED THE GSK IMPACT AWARD FOR EXCELLENCE IN BUILDING A HEALTHIER COMMUNITY.
