GRANDFAMILY CONNECT, A SERVICE PROVIDED BY GRANDFAMILY RESOURCE CENTER. PHILLY FAMILIES CONNECT PROVIDE GRANDPARENTS WITH KNOWLEDGE AND SUPPORT TO NAVIGATE THE CHALLENGES OF LEARNING, INCREASE THEIR UNDERSTANDING OF THE CHILDREN'S LEARNING NEEDS AND AVAILABLE SUPPORTS, AND PROVIDE A VENUE TO FIND ENCOURAGEMENT, RESOURCES, AND STRATEGIES FROM PEERS AND PROFESSIONALS SERVICES PROVIDED BOTH IN PERSON AND VIA PHONE INCLUDE GROUP COUNSELING, EDUCATIONAL WORKSHOPS, INDIVIDUAL COUNSELING AND REFERRALS TO NEEDED SERVICES. GRANDFAMILY CONNECT REACHES OVER 70 FAMILIES WITH 105 CHILDREN.

PHILLY FAMILIES READ TOGETHER IS INTERGENERATIONAL PROGRAM FOR GRANDPARENTS ENRICHING THE EDUCATION OF THEIR GRANDCHILDREN. THE MAIN FOCUS OF THE PROGRAM IS TO IMPROVE LITERACY SKILLS FOR CHILDREN AND ALSO TO IMPROVE THE DIGITAL LITERACY SKILLS OF GRANDPARENTS. PHILLY FAMILIES READ TOGETHER PROVIDES WEEKLY FAMILY LITERACY WORKSHOPS. THESE WORKSHOPS TEACH GRANDPARENTS HOW TO HELP CHILDREN BUILD READING, WRITING, SPEAKING, AND LISTENING SKILLS. PFRT SERVES OVER 35 FAMILIES WITH 50+ INDIVIDUALS.

HOMEBOUND GROUP TELEPHONE COUNSELING IS AN EFFECTIVE SERVICE DELIVERY MODEL TO PROVIDE SERVICES TO THOSE ISOLATED AT HOME. PROVIDING ESSENTIAL SUPPORT AND 'SAFETY NET' SERVICES WITH WEEKLY CONTACT TO IDENTIFY ISSUES, DISCUSS SOLUTIONS AND ACCESSING SERVICES BEFORE A CRISIS OCCURS. GIVEN THE USE OF THE TELEPHONE, THE SERVICE IS PORTABLE, ENABLING SEAMLESS SERVICE ACROSS PHYSICAL LOCATION SETTINGS. THESE GROUPS ARE FACILITATED BY LICENSED CLINICAL SOCIAL WORKERS. PARKINSON'S DISEASE CARE PARTNERS PROGRAM IS A TELEPHONE-BASED PROGRAM TO SUPPORT CAREGIVERS WITH FAMILY MEMBER WHO HAS PARKINSON'S DISEASE. PROGRAM PROVIDES EDUCATION, SUPPORT AND PEERS AS THEY WALK THE JOURNEY OF CARING FOR FAMILY MEMBER WITH PARKINSON'S DISEASE. PARKINSON'S DISEASE CARE PARTNERS PROGRAM PROVIDES SERVICES TO 30 CAREGIVERS.
