LEHIGH VALLEY HEALTH NETWORK (LVHN) REHABILITATION CENTER SCHUYLKILL BRINGS TOGETHER A FULL RANGE OF SERVICES IN A TEAM APPROACH TO TREAT NEUROLOGICAL AND ORTHOPEDIC DISORDERS. OUR EXPERIENCED TEAM IS COMMITTED TO PROVIDING QUALITY REHABILITATION. STATE-OF-THE ART FACILITIES AND EQUIPMENT ALLOW THIS DEDICATED TEAM TO FOCUS ON RESULTS. SERVICES INCLUDE: PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY WITH SPECIALTY PROGRAMS OF ORTHOPEDICS, NEUROLOGICAL, AQUATICS, HAND THERAPY, LYMPHEDEMA MANAGEMENT, CANCER REHAB, VESTIBULAR REHAB AND WORK CONDITIONING PROGRAM AS WELL AS SUPPORT OF PSYCHOLOGY SERVICES. IN 2019 THERE WERE 1504 ADMISSIONS AT THE REHAB CENTER, WITH ACCESS TO THE FACILITY 0.91% OVER THE NATIONAL BENCHMARK ALONG WITH AVAILABILITY TO A CONVENIENT APPOINTMENT, .98% OVER THE NATIONAL BENCHMARK COMPILED BY FOTO (FOCUS ON THERAPEUTIC OUTCOMES NATIONAL) SURVEY SYSTEM. OVERALL PATIENT SATISFACTION SCORE WAS 98.12% VS. THE NATIONAL BENCHMARK OF 97.93% WITHIN THIS SAME SYSTEM DEMONSTRATING THE COMMITMENT TO PATIENT ENGAGEMENT. LVHN REHABILITATION CENTER SCHUYLKILL'S LEVEL OF DURABILITY OF TREATMENT WAS ALSO RANKED HIGH IN 2019. IN SUMMARY, 96.5% OF THE PERSONS SERVED AT SRC REPORTED THEIR OVERALL FUNCTIONAL STATUS IMPROVED OR STAYED THE SAME SINCE THEIR DISCHARGE 90 DAYS PRIOR. RESPONSES RECEIVED DEMONSTRATED THAT 98.4% OF THE PERSONS SERVED IN THE COMMUNITY WOULD RECOMMEND OR USE THE SERVICES AT THE REHAB CENTER. THE CENTER ALSO CONTINUED TO MEET THE ESTEEMED STANDARDS OF CARF (COMMISSION ON ACCREDITATION OF REHABILITATION FACILITIES) COMPLETING A RENEWAL SURVEY IN SEPTEMBER OF 2019 WHICH RESULTED IN 3-YEAR ACCREDITATION RANKING IN THE TOP 3% INTERNATIONALLY PROVIDING QUALITY SERVICE DELIVERY.THE PROGRAMMING AND STAFF AT THE REHAB CENTER ARE COMMITTED TO THE COMMUNITY IN PROVIDING EDUCATION AND SUPPORT. IN 2019, THE STAFF AT LVHN REHABILITATION CENTER SCHUYLKILL PROVIDED MONTHLY SUPPORT GROUP MEETINGS OF PARKINSON'S, MS, FIBROMYALGIA/CHRONIC PAIN, TBI/STROKE AND AMPUTEE. PREVENTATIVE INFORMATION RELATED TO SAFETY AND BODY MECHANICS WAS PRESENTED AT HEALTH AND WELLNESS FAIR LOCALLY IN THE COMMUNITY AT TREDEGAR FILMS.LEHIGH VALLEY HEALTH NETWORK (LVHN) REHABILITATION CENTER - SCHUYLKILL'S PROCESS IMPROVEMENT PLAN WAS DEVELOPED THROUGH ALL THE FACILITY'S COMMITTEES ALONG WITH THE MARKETING PLANS AND FISCAL AUDIT INCENTIVES. THE COMMITTEES INCLUDE PROGRAM QUALITY ASSESSMENT & PEER REVIEW (PQAPR), INFORMATION AND OUTCOMES MANAGEMENT (IOMS), SAFETY, EQUIPMENT, INFECTION CONTROL, ACCESSIBILITY AND CONTINUING EDUCATION. EACH COMMITTEE'S CHAIRPERSON HIGHLIGHTED THEIR ACCOMPLISHMENTS FOR THE YEAR INTEGRATED INTO A FACILITY PLAN FOR IMPROVEMENT IN THE COMING FISCAL YEAR. SOME OF THE HIGHLIGHTED ACCOMPLISHMENTS WERE AS FOLLOWS:THE INFORMATION AND OUTCOMES MANAGEMENT SYSTEM (IOMS) COMMITTEE ACHIEVED ITS GOAL FOR OVERALL PATIENT SATISFACTION PERCENTILE RANKING TO EXCEED THE 50TH PERCENTILE (OPTIMAL TO 70TH PERCENTILE). THE RANKING WAS AT 65% IN 2019 WELL ABOVE THE ESTABLISHED GOAL LEVEL. THE IOMS COMMITTEE'S GOAL TO IMPROVE THE FUNCTIONAL STATUS CHANGE RATE TO NATIONAL FOTO BENCHMARK WAS PARTIALLY MET THIS PAST YEAR. THIS GOAL IS TO MEET OR EXCEED THE BENCHMARK FOR EACH OF THE 12 CATEGORIES, 7OF 12 CATEGORIES EITHER MET OR EXCEEDED FOTO BENCHMARK (58%); THE REMAINING 5 CATEGORIES WERE WITHIN 1-2 OF BENCHMARK.THE ACCESSIBILITY COMMITTEE CONTINUALLY UPDATED THE COMMUNITY RESOURCE BINDER WITH CURRENT ACCESSIBLE FACILITIES/RECREATIONAL ACTIVITIES/INFORMATION FOR THOSE INDIVIDUALS WITH A DISABILITY. COMMUNITY EVENTS INCLUDED PROVIDING INFORMATION ON REHABILITATION SERVICES AVAILABLE AT HERITAGE DAYS IN POTTSVILLE, ORWIGSBURG, SCHUYLKILL HAVEN AND TAMAQUA, PARTICIPATION IN DAIKON LIVING AND LEARNING AFTER 50 EXPO IN ST. CLAIR AND OPIOID LECTURE AT PENN STATE SCHUYLKILL CAMPUS, AND COLLABORATION WITH LVPG ORTHOPEDIC SURGEON DR. HELMOLD TO PROVIDE A HAND AND WRIST PAIN SCREENING FOR INDIVIDUALS WITHIN OUR LOCAL COMMUNITY CONSISTING OF LECTURE AND ASSESSMENT SCREENING.
