Organizations Filed Purposes:
Our mission is to provide programs and services to older adults so they can age in their homes safely and independently; Programs and services for older adults are provided in Cameron, Elk and McKean Counties: Insurance Counseling, Domiciliary Care, Caregiver Support Programs, Information and Referral services, Care Management, Ombudsman, OPTIONS Personal Care, Protective Services, Nutrition Services and PDA Health and Wellness programs in 13 Senior Centers
Human Services Program:OHS purchased 302 daily visits in a community adult day service program for 7 older adults.OHS performed 566 financial eligibility determination assessments for older adults in need of in home care/nursing facility care services.OHS purchased 47,541 congregate meals for 1,141 older adults provided at 13 Senior Centers in Cameron, Elk and McKean counties.OHS reimbursed 126 caregivers $125,431 in the Caregiver Support Program to help them financially provide care for an older adult.OHS purchased 1,852 home delivered meals for 28 older adultsOHS provided 24,359 callers with information and referral assistance service.OHS performed 221 protective service investigations on 124 older adults alleged to be victims of abuse, neglect or exploitation.OHS received and responded to 293 reports of need for the protection of adults.OHS provided 860 older adults with social, recreational, educational and health and wellness programs at 13 Senior Centers in Cameron, Elk and McKean counties.
Personal Care:OHS purchased 64,591 hours of personal care to assist 404 older adults. OHS purchased 1,149 hours of medication management to assist 57 older adults.OHS reimbursed 92 older adults for 648 monthly fees for personal emergency response units.
Care Management: OHS provided 650 hours of care management services to older adults which included 7,761 assessments of need that support older adults staying in their home safely and independently.OHS transitioned 2 nursing home residents safely back into their homes. This service is no longer offered by OHS effective August 31, 2019; it is being provided by the Managed Care Companies contracted by the Department of Human Services.OHS provided 1,044 hours of service coordination care management to older adults in the Aging Waiver program who are clinically eligible for nursing facility care. We helped those older adults stay in their homes and released from a nursing home. This service is no longer provided by OHS as of August 31, 2019; it is being provided by the Managed Care Companies contracted by the Department of Human Services.
