Organizations Filed Purposes: TO LINK PEOPLE LIVING WITH HIV TO RESOURCES FOR LEADING HEALTHY, SELF-DIRECTED LIVES.

FOOD SERVICES: INCLUDES AN ON-SITE MEAL PROGRAM THAT SERVED 25,800 MEALS IN 2018. HIGHLIGHTS: A HEALTHY FOODS POLICY MEANS THE FOOD SHELF AND MEAL PROGRAM ARE PROCURING AND SOURCING INGREDIENTS WITH MAXIMUM NUTRITION BENEFITS FOR OUR MEMBERS. THE REGISTERED DIETITIANS WORKED WITH 332 MEMBERS TO HELP THEM WORK TOWARD NUTRITION-RELATED GOALS. 83 TONS OF GROCERIES WERE DISPENSED FROM THE FOOD SHELF."

MEMBER SERVICES: INCLUDES A MEDICAL CASE MANAGEMENT PROGRAM WHICH SERVED 191 MEMBERS AND A NON-MEDICAL CASE MANAGEMENT PROGRAM WHICH SERVED 107 MEMBERS. HIGHLIGHTS: WE ADDED 220 NEW MEMBERS IN 2018. CASE MANAGEMENT LINKED 298 MEMBERS TO RESOURCES. WE ADDED A NEW HOUSING PROGRAM WHICH SERVED 93 MEMBERS AND SUCCESSFULLY HOUSED 41. WE GAVE OUT 478 TICKETS TO 32 DIFFERENT EVENTS, AND PROVIDED HOLIDAY ITEMS TO 291 MEMBERS AND THEIR FAMILIES. OTHER REGULAR ACTIVITIES IN THE SPACE CONTINUED, INCLUDING TRIVIA, BINGO, COMMUNITY CONVERSATIONS.

OUTREACH AND EDUCATION: INCLUDES A HEALTH AND WELLNESS PROGRAM THAT OFFERS EDUCATIONAL SEMINARS, OUTREACH SERVICES, HIV TESTING AND A RESOURCE LIBRARY WITH INTERNET ACCESS. HIGHLIGHTS: OUTREACH AND CARE LINKAGE CONTINUES TO GROW AND FOCUS ON AT-RISK POPULATIONS INCLUDING AFRICAN BORN AND LATINX COMMUNITIES. 563 HIV TESTS WERE ADMINISTERED TO HELP PEOPLE KNOW THEIR HIV STATUS AND 28 PEOPLE OUT OF CARE WERE LINKED TO MEDICAL CARE. A PARTNERSHIP WITH PRIDE INSTITUTE IS REACHING THE HIV COMMUNITY ON A NATIONAL LEVEL. WE ARE DISTRIBUTING NALOXONE INTO THE COMMUNITY. ALIVENESS IS THE PRIMARY SITE FOR HIV COORDINATED ENTRY FOR HENNEPIN COUNTY. OPEN DAILY (9AM-4PM) TESTING DURING ALIVENESS BUSINESS HOURS. 8 HIV EDUCATION WORKSHOPS WERE HELD FOR ALIVENESS MEMBERS, THEIR LOVED ONES, VOLUNTEERS, AND STAFF. AN AVERAGE OF 23 PEOPLE ATTENDED EACH SEMINAR AND 91 UNDUPLICATED MEMBERS WERE SERVED. TOPICS INCLUDED PREVENTION (U=U AND MORE), THE EVOLUTION OF HIV TREATMENT (INCLUDING NEW RESEARCH), AGING WITH HIV, WOMEN AND HIV, AND PEER ADVOCACY. OUR HEALTH & WELLNESS PROGRAM PROVIDED 1,034 INTEGRATIVE THERAPIES SESSIONS TO 229 MEMBERS.
