THE BRIDGE ALSO HOSTS YSNMN.ORG, A MOBILE APP ALLOWING YOUTH TO SEARCH FOR AND CONNECT WITH AVAILABLE SHELTER AND RESOURCES AT 53 LOCAL AND NATIONAL ORGANIZATIONS SERVING MINNESOTA YOUTH.IN FY20, THERE WERE 1,727 CRISIS CALLS AND REFERRALS; 401 CRISIS TEXT MESSAGES; 1,289 VISITORS ACCESSING BFY THROUGH YSNMN.ORG; 88,438 VISITS TO YSNMN.ORG APP. HC365 - CASE MANAGEMENT SUPPORT FOR HOMELESS YOUTH AS THEY TRANSITION TO LIVING INDEPENDENTLY; THIS INCLUDES SECURING SAFE AND STABLE HOUSING, ASSISTANCE IN OBTAINING LEGAL DOCUMENTS, INDEPENDENT LIVING SKILLS BUDGETING, COOKING, EMPLOYMENT, ETC,. OUTREACH - THE OUTREACH TEAM CONNECTS WITH YOUTH, OFFERS EDUCATIONAL PRESENTATIONS AND IS A PRESENCE AT COMMUNITY EVENTS TO PROMOTE AWARENESS OF THE BRIDGE SERVICES AND RESOURCES. WORK AMBASSADOR - EMPLOYMENT AND CASE MANAGEMENT SUPPORT FOR HOMELESS YOUTH THAT PROVIDES THE OPPORTUNITY TO GAIN WORK EXPERIENCE, DEVELOP RESUMES AND PRACTICE INTERVIEW SKILLS. JUVENILE DIVERSION - SHELTER AND LONG-TERM HOUSING OPTIONS FOR FIRST-TIME OFFENDERS AND CORRECTIONS-INVOLVED YOUTH. WEEKLY SUPPORT GROUPS - SUPPORT YOUTH IN BUILDING COMMUNITY AND STAYING ON TRACK WITH THEIR GOALS. GROUPS INCLUDE CRIME VICTIMS, LGBTQ, AND WORK READINESS. JUVENILE DIVERSION - SHELTER AND LONG-TERM HOUSING OPTIONS FOR FIRST-TIME OFFENDERS AND CORRECTIONS-INVOLVED YOUTHWEEKLY SUPPORT GROUPS - SUPPORT YOUTH IN BUILDING COMMUNITY AND STAYING ON TRACK WITH THEIR GOALS. GROUPS INCLUDE CRIME VICTIMS AND LGBTQ.

"PROGRAM" - RESILIENCE HOUSE (EMERGENCY SHELTER PROGRAM) "THE EMERGENCY SHELTER IS OPEN 24/7/365. FOCUSED ON FAMILY REUNIFICATION WHENEVER SAFEAND POSSIBLE, THE EMERGENCY SHELTER PROVIDES TEMPORARY SHELTER, MEALS, ACCESS TO HEALTHCARE, CASEMANAGEMENT, AND SUPPORT FOR YOUTH AGES 10-17. IN FY20: 330 YOUTH SHELTER STAYS 457 SUPPORT GROUP ATTENDEES; 85% OF YOUTH EXITED TO SAFE AND STABLE HOUSING, WITH 53% REUNITED WITH FAMILY; 32% ALTERNATIVE SAFE EXITS; 96% COMPLETED GOALS AFTER ORIENTATION WHILE IN SHELTER. THE AVERAGE LENGTH OF STAY WAS 9 DAYS.IN LATE AUGUST, GLORIA'S PLACE OPENED, PROVIDING EMERGENCY SHELTER, FOOD AND CLOTHING FOR UP TO SIX HOMELESS TEEN PARENTS (AGES 15-17) AND THEIR CHILDREN (AGES 0-3).

"PROGRAM" - SUPPORTIVE HOUSING "RITA'S HOUSE - OUR PROPERTY ON 2200 EMERSON AVENUE, AN INTENTIONAL LIVING COMMUNITY, PROVIDING AFFORDABLE RENTAL HOUSING FOR 18 TO 21 YEAR OLD YOUTH. WHILE LIVING IN THE HOUSE, THE YOUNG ADULTS LEARN INDEPENDENT SKILLS, WORK WITH A CASE MANAGER AND BUILD A RENTAL HISTORY. IN FY20, 88% OF THE RESIDENTS HAD SAFE EXITS AND 100% HAD A CASE PLAN, OF WHICH 100% HAVE COMPLETED AT LEAST ONE GOAL. THE AVERAGE LENGTH OF STAY IS 461 DAYS. MARLENE'S PLACE -THE FIRST AND ONLY SITE-BASED TRANSITIONAL HOUSING AND CASE MANAGEMENT PROGRAM IN HENNEPIN COUNTY FOR HOMELESS, PREGNANT/PARENTING TEENS (AGES 16-20) AND THEIR CHILDREN (AGES 0-3). IN FY20, FOURTEEN YOUTH RESIDED IN MARLENE'S PLACE, WITH 82% AVERAGE UTILIZATION RATE, 100% OF TEENS RECEIVING PARENTING EDUCATION, AND, 100% OF YOUTH ENROLLED IN SCHOOL OR WORKING.
