CHCM PARTNERS WITH EMPLOYERS TO MAKE AN IMPACT ON THE HEALTH OF THEIR EMPLOYEES, CUSTOMERS AND COMMUNITY BY ACTING AS THE CONDUIT FOR INDIVIDUALS IN THE WORKPLACE TO ENGAGE IN HEALTH CAUSES THEY CARE ABOUT THROUGH EDUCATION, GIVING AND VOLUNTEERISM.

TO IMPROVE THE HEALTH OF MINNESOTANS AFFECTED BY CHRONIC ILLNESS.

ALLOCATIONS- FINANCIAL ALLOCATIONS ARE DISTRIBUTED TO MEMBER HEALTH CHARITIES ON A QUARTERLY BASIS. SMALL DISTRIBUTIONS ARE MADE TO NON-MEMBER CHARITIES PER EMPLOYEES' DESIGNATION. ALLOCATIONS ARE BASED ON DESIGNATED AND UNDESIGNATED CONTRIBUTIONS FROM EMPLOYEE GIVING PROGRAMS. UNDESIGNATED CONTRIBUTIONS ARE ALLOCATED ACCORDING TO MEMBER CHARITY'S PARTICIPATION IN CHCM PROGRAMS SUCH AS HEALTH MATTERS AT WORK, SPEAKER'S BUREAU, AND CAMPAIGN DEVELOPMENT.

PROMOTION OF MEMBER CHARITIES' PROGRAMSYEAR-ROUND WORKPLACE PROMOTION OF MEMBER CHARITIES' HEALTH EDUCATION PROGRAMS, DIRECT SERVICES AND RESOURCES:* HEALTH MATTERS AT WORK: ONLINE RESOURCES AND ONSITE HEALTH EDUCATION SEMINARS WITH 60 PROGRAMS ON HEALTH AND CHRONIC ILLNESS: 30 ON-SITE PROGRAMS COMPLETED IN FY 2014, DIRECTLY EDUCATING 1,420 INDIVIDUALS; 14 ONLINE PROGRAMS COMPLETED IN FY 2014, DIRECTLY EDUCATING MORE THAN 160 INDIVIDUALS.* HEALTH AND INFORMATION FAIRS: HEALTH SCREENINGS, INFORMATION AND RESOURCES AT WORKPLACE HEALTH AND WELLNESS FAIRS; 28 HEALTH AND INFORMATION FAIRS PRESENTED IN FY 2014.* INSPIRATIONAL SPEAKERS: CHCM RECRUITS AND TRAINS VOLUNTEER REPRESENTATIVES FROM MEMBER CHARITIES TO SHARE THEIR PERSONAL STORIES. SPEAKERS PRESENTED AT 57 SPEAKING ENGAGEMENTS IN FY 2014.* CHCM WEBSITE: FEATURES MEMBER CHARITY INFORMATION, RESOURCES AND VOLUNTEER OPPORTUNITIES.
