TOUCHSTONE'S RISING CEDAR APARTMENTS IS A UNIQUE, 40 ONE-BEDROOM UNIT ASSISTED LIVING APARTMENT BUILDING OFFERING STATE-OF-THE-ART CONSTRUCTION SPECIFICALLY DESIGNED AS A HEALING ENVIRONMENT TO ENHANCE HEALTH AND WELLNESS FOR ADULTS. RISING CEDAR APARTMENTS OFFER CO-CREATED MENTAL HEALTH SERVICES SUCH AS ILLNESS AND SYMPTOM MANAGEMENT SKILLS ASSISTANCE, INDEPENDENT LIVING SKILLS ASSISTANCE, MEDICATION ADMINISTRATION, HEALTH CARE COORDINATION, AND MORE. ASSISTED LIVING, NEW HOPE IS MINNESOTA'S FIRST LONG-TERM ASSISTED LIVING PROGRAM FOR ADULTS WITH MENTAL ILLNESS AND HAS BEEN IN OPERATION FOR OVER 15 YEARS. OUR AVERAGE LENGTH OF STAY FOR THE 19 ADULTS LIVING IN AT NEW HOPE'S PERMANENT SUPPORTIVE HOUSING MODEL IS OVER EIGHT YEARS.
TOUCHSTONE OPERATES THREE RESIDENTIAL TREATMENT PROGRAMS IN MINNEAPOLIS, BLOOMINGTON, AND FRIDLEY, MINNESOTA. THESE LOCATIONS OFFER INTENSIVE RESIDENTIAL TREATMENT SERVICES TO INDIVIDUALS WITH A MENTAL HEALTH DIAGNOSIS, WHO MAY ALSO BE STRUGGLING WITH SUBSTANCE USE DISORDERS. TREATMENT SERVICES ARE PROVIDED UP TO 90 DAYS, 24 HOURS A DAY, 7 DAYS A WEEK. TOUCHSTONE'S FRIDLEY LOCATION ALSO OFFERS CRISIS STABILIZATION SERVICES FOR UP TO 10 DAYS. THESE PROGRAMS PROVIDE A STRUCTURED, SUPPORTIVE, AND COMFORTABLE ENVIRONMENT FOCUSING ON INTEGRATIVE SERVICES INCLUDING MIND, BODY, AND SPIRIT. IN 2019, TOUCHSTONE MENTAL HEALTH RECEIVED 1,208 IRTS REFERRALS AND 1,165 CRISIS REFERRALS. THE PROGRAMS ADMITTED 214 IRTS RESIDENTS ACROSS ALL THREE SITES AND 222 CRISIS RESIDENTS AT THE FRIDLEY LOCATION.
TOUCHSTONE PROVIDES BEHAVIORAL HEALTH SERVICES THROUGH CASE MANAGEMENT, CARE COORDINATION, AND A BEHAVIORAL HEALTH HOME. THESE SERVICES PROVIDE FACE-TO-FACE, TELEPHONIC, AND COLLABORATIVE SUPPORT STARTING WITH A COMPREHENSIVE, CLIENT-CENTERED ASSESSMENT HIGHLIGHTING STRENGTHS AND NEEDS. THESE PROGRAMS PROVIDE DIRECT ASSISTANCE WITH HEALTH CARE NAVIGATION, ALONG WITH SOCIAL SERVICE SUPPORTS. THESE SERVICES PROVIDED SUPPORT TO APPROXIMATELY 1000 INDIVIDUALS IN 2019 AND WORKED TO REDUCE PSYCHIATRIC HOSPITALIZATIONS AND ENSURE ALL CLIENTS HAD ACCESS TO A PRIMARY CARE PHYSICIAN AND BEHAVIORAL HEALTH SERVICES.
