Organizations Filed Purposes: DEVELOP, COORDINATE AND PROVIDE SERVICES TO MEET THE NEEDS OF OLDER ADULTS/SUPPORT THEIR CAREGIVERS.

SENIOR CENTERS:FIVE AFFILIATED SENIOR CENTERS PROVIDED SERVICES TO 16,380 OLDER MINNESOTANS IN 2019. BY PARTNERING WITH SENIOR COMMUNITY SERVICES, THESE CENTERS ARE ABLE TO RECEIVE A WIDE ARRAY OF SUPPORTS, WHILE MAINTAINING THEIR ABILITY TO MEET THE UNIQUE NEEDS OF THEIR COMMUNITIES. EACH CENTER KEEPS OLDER ADULTS ENGAGED, HEALTHY AND ACTIVE IN THE COMMUNITY THROUGH A VARIETY OF INTELLECTUAL, SOCIAL AND PHYSICAL ACTIVITIES INCLUDING ART, FITNESS AND EDUCATIONAL CLASSES, CARD AND GARDEN CLUBS, COMMUNAL DINING, DAY TRIPS, BLOOD PRESSURE CHECKS AND OTHER HEALTH SERVICES. ADDITIONAL SERVICES IN 2020 WILL INCLUDE VIRTUAL PROGRAMMING. SERVICE AREA: HENNEPIN AND WRIGHT COUNTIES.

HOUSEHOLD & OUTSIDE MAINTENANCE (HOME) PROGRAM:THE HOME PROGRAM PROVIDED 1,321 LOW-INCOME OLDER ADULTS WITH HIGH-QUALITY HOUSEHOLD CHORE AND MAINTENANCE SERVICES DELIVERED BY TRUSTED PROFESSIONALS AND COMMUNITY VOLUNTEERS. AS THE LARGEST SENIOR CHORE PROVIDER IN MINNESOTA, WE PROVIDE THESE SERVICES ON A SLIDING-FEE SCALE AT AN AFFORDABLE COST. THE IMPACT OF THE HOME PROGRAM IS MANIFOLD, POSITIVELY AFFECTING THOSE WE SERVE, THOSE WHO VOLUNTEER THEIR TIME AND EFFORTS, AND THE COMMUNITY AT LARGE. ADDITIONAL SERVICES WILL INCLUDE TELEPHONE REASSURANCE CALLS AND A TECHNOLOGY SERVICE TO ASSIST OLDER ADULTS WITH TECHNOLOGY IN 2020. SERVICE AREA: HENNEPIN AND WRIGHT COUNTIES AND EXPANDING TO SHERBURNE COUNTY IN 2020.

SENIOR OUTREACH & CAREGIVER SERVICES PROGRAMS:THROUGH SERVICE COORDINATION, SUPPORTIVE COUNSELING, ADVOCACY AND CAREGIVER SERVICES, 759 OLDER ADULTS AND THEIR CAREGIVERS WERE HELPED BY HIGHLY SKILLED, LICENSED SOCIAL WORKERS TO AGE IN PLACE WITH DIGNITY, SAFETY, AND HEALTH, AND TO MAINTAIN BALANCE IN THEIR LIVES. SERVICES AREA: OLDER ADULTS IN SUBURBAN HENNEPIN COUNTY AND CAREGIVERS IN HENNEPIN, CARVER COUNTIES AND EXPANDING TO WRIGHT COUNTY IN 2020. CARENEXTION.ORG, THIS EFFICIENT ONLINE TOOL, ENABLED 9,335 CAREGIVERS TO COORDINATE TASKS, LOCATE RESOURCES, JOURNAL VISITS AND GET REAL-TIME HELP FOR THEMSELVES. SERVICE AREA: WORLDWIDE VIA INTERNET.MEDICARE PARTNERS, FORMERLY SENIOR PARTNERS CARE, HELPED 1,034 LOW-TO-MODERATE INCOME OLDER MINNESOTANS RECEIVE CARE FROM PARTICIPATING HEALTHCARE PROVIDERS, INCLUDING MOST MAJOR METRO-AREA HOSPITALS, AND HUNDREDS OF CLINICS AND PHYSICIANS, WHO HAVE AGREED TO ACCEPT MEDICARE PAYMENT IN FULL, WAIVING ALL MEDICARE CO-PAYS AND DEDUCTIBLES, SO THAT THOSE ON A FIXED INCOME CAN AVOID THE MENTAL AND EMOTIONAL STRESSES THAT STUDIES CONNECT WITH FINANCIAL HARDSHIP.
