DEVELOP AND COORDINATE COMMUNITY CARE SERVICES FOR OLDER PEOPLE.
GRANT MAKING: CENTRAL MN COUNCIL ON AGING IS A PIVOTAL LINK BETWEEN COMMUNITY SUPPORTS FOR SENIORS, STATE AND FEDERAL FUNDING FOR THOSE SERVICES IN ITS 14 COUNTY PLANNING AND SERVICE AREA. CMCOA AWARDS CONTRACTS FOR SERVICES TO HELP ENSURE THAT VAST ARRAYS OF OPTIONS ARE IN PLACE TO MATCH THE NEEDS OF THE COMMUNITY AND ALLOW SENIORS TO REMAIN LIVING INDEPENDENTLY IN THEIR COMMUNITY FOR AS LONG AS POSSIBLE. IN 2019, $2,732,667 PURCHASED SERVICES WITH THESE DOLLARS WHICH INCLUDED 782 HOURS OF COMPREHENSIVE HEALTH ASSESSMENT, 3,666 HOURS OF LEGAL ASSISTANCE, 1 LEGAL EDUCATION SESSION, 11,471 RIDES OF ASSISTED TRANSPORTATION, 1,425 HOURS OF CHORE SERVICES, 4,808 HOURS OF HOMEMAKER SERVICES, 622 HOURS OF SUPPORT PLANNING, 30 SERIES OF EVIDENCED BASED HEALTH PROMOTION PROGRAMS, 7,670 HOURS OF CAREGIVER IN-HOME/GROUP RESPITE, 1,973 HOURS OF CAREGIVER COUNSELING SERVICES, 70 CAREGIVER EDUCATION SESSIONS, 208,694 CONGREGATE MEALS AND 198,309 HOME DELIVERED MEALS. A TOTAL OF $511,225 WAS UTILIZED AS IN-KIND MATCH BY THE SERVICE PROVIDERS IN THE DELIVERY OF THE AFOREMENTIONED SERVICES.
SENIOR LINKAGE: THE SENIOR LINKAGE LINE IS AN IMPORTANT PART OF THE AGING SERVICES NETWORK. WITH ONE CALL TO THE SENIOR LINKAGE LINE, SENIORS, FAMILIES, CAREGIVERS, AGENCIES AND MEDICARE BENEFICIARIES OF ANY AGE CAN OBTAIN ASSISTANCE TO EVALUATE THEIR SITUATION AND LOCATE SERVICES THAT ARE AVAILABLE WITHIN THEIR COMMUNITIES TO HELP THEM REMAIN INDEPENDENT. IN 2019 SENIOR LINKAGE LINE SERVED 58,325 INDIVIDUALS WITH PHONE ASSISTANCE AND 6,641 WITH IN-PERSON ASSISTANCE. TWENTY-FIVE COMMUNITY ACCESS SITES WERE AVAILABLE TO OLDER ADULTS AND INDIVIDUALS ON MEDICARE IN 2019. 23 ACTIVE HEALTH INSURANCE COUNSELING VOLUNTEERS CONTRIBUTED 983 HOURS OF INSURANCE COUNSELING.
PROGRAM DEVELOPMENT: FEATURING FOUR KEY GOALS 1.) LEVERAGE THE EXPERIENCE, EXPERTISE, AND ENERGY OF OLDER MINNESOTANSLIVABLE COMMUNITIES:PROVIDED TECHNICAL ASSISTANCE (TA) TO TWO SILOS TO CIRCLES PILOT COMMUNITIES; CHISAGO AGE WELL IN SOUTHERAN CHISAGO COUNTY AND CUYUNA AREA CONNECTIONS IN CROSBY, MINNESOTA. THE SILOS TO CIRCLES AGE WELL INITIATIVE IS DESIGNED TO STRENGTHEN LOCAL CONNECTIONS TO AGING SERVICES FOR SENIORS, THEIR FAMILIES, AND NEIGHBORS. DURING THE COURSE OF THE PILOT , THE TWO COMMUNITIES COMBINED OFFERED 190 OUTREACH EVENTS, WITH 3,040 ATTENDEES. PRESENTED HIGHLIGHTS OF THE SILOS TO CIRCLES COMMUNITIES AT THE AGE & DISABILITY CONFERENCE AND BLANDIN FOUNDATION BROADBAND CONFERENCE.AS A FOLLOW-UP TO 2018 CITY SURVEYS, BEGAN MEETING WITH CITY LEADERS TO DISCUSS THE NEEDS OF THEIR COMMUNITY AND START THE DISCUSSION OF AGE-FRIENDLY COMMUNITIES.SOCIAL ISOLATION:PARTNERED WITH AMBER CARROLL, COVIA TO OFFER A PRESENTATION ON WELL CONNECTED AT THE AGE & DISABILITY CONFERENCE. ALSO PROMOTED IT AT COMMUNITY EVENTS AND MEETINGS.WORKFORCE:PROVIDED PRESENTATIONS TO BUSINESSES AND CHAMBER GROUPS TO EXPAND KNOWLEDGE AND THOUGHTS ABOUT OLDER ADULTS AND CAREGIVERS IN THE WORKFORCE. ENCOURAGING EMPLOYERS TO EMBRACE THE EXPERTISE OF OLDER ADULTS AND ALSO LOOK AT THE NEEDS OF EMPLOYEES THAT ARE CARING FOR A LOVED ONE. PARTNERED WITH REGION 3 LEADERSHIP AND PLANNING BOARD, CENTRAL MN JOBS AND TRAINING SERVICES, INC., CAREER SOLUTIONS WORKFORCE DEVELOPMENT BOARDS, AND THE INITIATIVE FOUNDATION AS A PARTICIPANT IN A 1-DAY WORKSHOP, UNTAPPED WORKFORCE, KEEPING OLDER WORKERS IN THE LABOR FORCE. SERVED ON INNOVATION TEAM FOR REGION 7E. SERVED ON VETERANS SUICIDE PREVENTION COALITION IN THE ST. CLOUD AREA. INCREASED CONNECTIONS AND POTENTIAL PARTNERSHIPS WITH MENTAL HEALTH PROVIDERS TO TACKLE THE GAPS IN MENTAL HEALTH SERVICES FOR OLDER ADULTS AND THOSE ON MEDICARE. PROVIDED PRESENTATION ON OLDER ADULTS AND MENTAL HEALTH AT MN CRISIS INTERVENTION TEAM TRAINING TO LAW ENFORCEMENT OFFICERS AND FIRST RESPONDERS. PROVIDED PRESENTATION ON CMCOA SERVICES AND PRIORITY PROJECTS TO THE CENTRAL MN MENTAL HEALTH CENTER FOR THE MONTICELLO OFFICE. EDUCATED COMMUNITY PROVIDERS ON THE AGING MASTERY PROGRAM (AMP). ASSISTED TO RECRUIT INTERESTED INDIVIDUALS/AGENCIES TO BE TRAINED TO OFFER AMP IN THEIR COMMUNITIES. 4.) PROVIDED HOMELESS PRESENTATION TO CMCOA ADVISORY COMMITTEE AND CMCOA STAFF. REVIEWED BARRIERS AND WHAT HOMELESSNESS LOOKS LIKE IN RURAL COMMUNITIES. HELD A HALF-DAY SESSION ON ASSISTED LIVING INMINNESOTA: NEW FRAMEWORK & CONSUMER PROTECTIONS , WITH OVER 50 AGING SERVICES PROVIDERS AND ADVOCATES ATTENDING. THIS SESSION PROVIDED AN OVERVIEW OF THE NEW REGULATORY FRAMEWORK FOR ASSISTED LIVING IN MINNESOTA AND PROVIDED AN OPPORTUNITY FOR PARTICIPANTS TO PROVIDE FEEDBACK INTO THE DEVELOPMENT OF THE ASSISTED LIVING REPORTCARD.
