BREAKING CYCLES FOR AT-RISK FAMILIES AND CHILDREN FOR TOTAL FAMILY RECOVERY.
SUPPORTIVE HOUSING PROGRAM:PROGRAM GOAL: TO RETURN HOMELESS, RECOVERING WOMEN AND THEIR CHILDREN BACK TO SOCIAL AND ECONOMIC MAINSTREAM.TARGET POPULATION:HOMELESS/RECOVERING/DUAL-DIAGNOSED WOMEN AND THEIR CHILDREN REFERRED BY HENNEPIN COUNTY SHELTER. OUR WRAP AROUND MODEL INCLUDES FULLY FURNISHED APARTMENTS, CASE MANAGERS, CHILD CARE WORKERS, ONSITE PROGRAM LIAISONS, TRANSPORTATION AND AN ARRAY OF AUXILIARY PROGRAMS. IN 2018, 71 WOMEN AND 104 CHILDREN WERE SERVED. 100% MOVED TO PERMANENT HOUSING AT MOVE-OUT.
KIDS CONNECTION:PROGRAM GOAL: TO INCREASE THE ACADEMIC, SOCIAL AND HEALTH OUTCOMES OF HOMELESS AND AT-RISK CHILDREN FROM PRE-SCHOOL THROUGH MIDDLE SCHOOL. TARGET POPULATION: HOMELESS AND AT-RISK CHILDREN KINDERGARTEN-8TH GRADE LIVING WITHIN THE ST. LOUIS PARK SCHOOL DISTRICT. THE PROGRAM SERVED 125 CHILDREN IN 2018. THIS COMPREHENSIVE "OUT OF SCHOOL PROGRAM" IS TAUGHT BY FOUR LICENSED TEACHERS AND OVER 50 TRAINED VOLUNTEERS. THE PROGRAM IS OPERATIONAL FIVE DAYS PER WEEK AND 12 MONTHS DURING THE YEAR, AND HAS THE FOLLOWING COMPONENTS:- REACH FOR THE STARS - PROVIDES ELEMENTARY AGE CHILDREN WITH EXTENDED-DAY ACADEMIC ENRICHMENT.- SUMMER EXPLORERS - PROVIDES SUMMER SCHOOL EDUCATION AND RECREATION THROUGHOUT THE SUMMER MONTHSOUTCOMES:- MAINTAINED OR IMPROVED THEIR ACADEMIC PERFORMANCE: 93%- RETENTION FROM SUMMER PROGRAM TO SCHOOL YEAR: 70%
CLINICAL SERVICES:PROGRAM GOAL: TO PROVIDE INTEGRATED PRIMARY HEALTH, SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES TO SUPPORTIVE HOUSING CLIENTS FOR THE PURPOSE OF INCREASING THE CLIENT'S STABILIZATION AND LONG TERM OUTCOMES. TARGET POPULATION: CLIENTS LIVING WITHIN PERSPECTIVES SUPPORTIVE HOUSING PROGRAM AND REFERRED COMMUNITY MEMBERS. THE PROGRAM IS RUN UNDER THE SUPERVISION OF OUR CLINICAL DIRECTOR AND EMPLOYS FOUR MENTAL HEALTH PRACTITIONERS/PROFESSIONALS. IN 2018, 3000+ HOURS OF CLINICAL SERVICES WERE PROVIDED TO ADULTS AND CHILDREN.THE COMPONENTS ARE:- ADULT SERVICES: CHEMICAL AND MENTAL HEALTH SERVICES INCLUDE ASSESSMENTS, DIAGNOSIS, TREATMENT PLANS AND THERAPY. ORGANIZATION IS A CO-OCCURRING RULE 245G OUTPATIENT CHEMICAL DEPENDENCY PROVIDER. THE PURPOSE OF THE PROGRAM IS TO INCREASE THE SUPPORTIVE HOUSING CLIENT'S STABILIZATION AND LONG TERM OUTCOMES.- CHILDREN'S MENTAL HEALTH SERVICES: INCLUDE ASSESSMENTS, DIAGNOSIS, TREATMENT PLANNING, SCREENING, REFERRALS AND THERAPY.
