TO HELP PEOPLE LIVE THEIR GREATEST LIVES!

FAMILY SUPPORTS - ACCORD'S FAMILY SUPPORTS SERVICES IS A WAIVER-FUNDED PROGRAM THAT'S ULTIMATE GOAL IS TO INCREASE INDEPENDENCE AND COMMUNITY CONNECTIONS FOR INDIVIDUALS WITH DISABILITIES. ACCORD OFFERS FOUR TYPES OF FAMILY SUPPORTS: PERSONAL SUPPORTS, RESPITE CARE, HOMEMAKER SERVICES, AND NIGHT SUPERVISION SERVICES. INDIVIDUALS CAN CHOOSE THE STAFF THAT SUPPORT THEM. THIS SERVICE IN 2020 WAS IMPACTED BY COVID DUE TO ITS IN HOME SUPPORT ASPECT. HOWEVER, WITH THE IMPACT, WE REMAINED STRONG WITH SERVING 230 FAMILIES AND INDIVIDUALS.

CASE MANAGEMENT - WE PROVIDE CASE MANAGEMENT AND SUPPORT SERVICES FOR INDIVIDUALS WHO QUALIFY UNDER THE DEVELOPMENTAL DISABILITIES (DD) WAIVER, COMMUNITY ALTERNATIVES FOR DISABLES INDIVIDUALS (CADI) WAIVER, AND THE BRAIN INJURY (BI) WAIVER. OUR CASE MANAGERS WORK WITH PEOPLE TO COMPLETE ASSESSMENTS, IMPLEMENT AN INDIVIDUAL SERVICE PLAN, ARRANGE AND MONITOR WAIVER ELIGIBLE SERVICES, AND CONSULT AND ADVOCATE ON BEHALF OF THE INDIVIDUAL. THROUGHOUT 2020 OUR CASE MANAGEMENT TEAM WAS ABLE TO PIVOT AND USE INNOVATION TO CREATE GREATER ACCESS FOR THE 2,171 PEOPLE WE SUPPORT WITH TELEHEALTH AND VIRTUAL VISITS.

EMPLOYMENT AND DAY SUPPORTS - WE HELP PEOPLE WITH DISABILITIES FIND EMPLOYMENT OPPORTUNITIES WITHIN THEIR COMMUNITIES. OUR ASSISTANCE HELPS THEM CONNECT WITH EMPLOYERS AND THEN MAINTAIN AND GROW THOSE ROLES. OUR DISCOVERY-BASED EMPLOYMENT PROCESS FINDS THE RIGHT MATCH FOR EACH INDIVIDUAL TO A JOB IN WHICH THEY CAN USE THEIR SKILLS, LEARN NEW ONES, AND EXPAND THEIR HORIZONS. WE RECOGNIZE THAT EMPLOYMENT ISN'T FOR EVERYONE AND ALSO PROVIDE DAY SUPPORTS AT TWO LOCATIONS FOR INDIVIDUALS TO ENJOY THEIR DAY THROUGH VARIOUS ACTIVITIES. WITH THE COVID-19 PANDEMIC, THESE TWO SERVICES WERE DIRECTLY IMPACTED THROUGH CLOSURES ALONG WITH UNEMPLOYMENT RATES. HOWEVER, THROUGH THOSE IMPACTS, WE WERE ABLE TO SUPPORT 448 INDIVIDUALS THROUGH THE EVER CHANGING LANDSCAPE OF 2020.
