Organizations Filed Purposes: CREATING OCCASIONS AND RELATIONSHIPS FOR THE INEVITABILITY OF CHANGE, ADDRESSING THE CHALLENGES OF RACISM AND POVERTY THROUGH THE DEVELOPMENT OF BOTH HUMAN CAPITAL AND COMMUNITY BUILIDNG BLOCKS.

A. Program: The community school collaborative (CSC) is an in-school and in-community partnership with Guadalupe Alternative Programs (GAP) focusing on 26 schools in the twin cities. Services include targeted outreach, social and emotional skill development, mediation, mentoring, parent engagement, case consultation, care coordination, staff development, Individual/Group/Family therapy.Services are provided year round during days and evenings, Monday - Friday. Roughly 700 children and families are served yearly.Goals: 80% of clients will report satisfaction with services upon termination of therapy. 80% of clients will report their problem as improved as rated by PSSF. 70% of clients will remain in the same placement/setting or move to a less restrictive placement/setting. 70% of clients will maintain or improve their functioning at home, school and in the community. B. Program: The clinic project is an outpatient therapy program. Clinicians are available to treat a variety of mental health concerns. Most major health plans, medical assistance and private pay are accepted as payment for outpatient therapy through the clinic project. Services are provided year round during days and evenings, Monday - Friday. Roughly 50 children and families are served yearly.Goals: Referrals will be contacted within one week of referral of being received. 90% of clients will report satisfaction with services upon termination of therapy. 80% of clients will report their problem as improved as rated by PSSF. C. Program: Mental Health Workforce Development includes supervision and training of mental health behavioral aides, mental health practitioners, and mental health professionals working towards licensure. Change Inc. partners with area graduate schools and with the Department of Human Services to train and supervise clinicians from ethnic and cultural minority communities. Services are provided Monday - Friday during working hours.An estimated 50 Mental Health Behavioral Aides, Practitioners and Professionals are trained and supervised yearly.Goals: Recruit, train and supervise 50 Mental Health Behavioral Aides, Practitioners, and Professionals yearly. Outreach, recruit, retain 15 Mental Health Behavioral Aides, Practitioners and Professionals from Ethnic and Cultural Minority Communities yearly. 90% of supervisees will progress to next Professional level within 2 year's time. 90% of Supervisees will rate supervision as good/excellent on yearly rating scale.D. Program: Change strategies & conversations at the crossroads include organizational and leadership development, consultation, research and training - specializing in multi-organizational collaboration and promoting mutual respect, learning and accountability. The Relational Climate Assessment, A Change Inc. developed tool, is utilized to assist organizations, schools and communities to focus on capacity and effectiveness of relational functioning. Conversations at the Crossroads is a series of events providing 'insider' views and community dialog on issues effecting Minneapolis and St. Paul.Services are provided as contracted.An estimate 3-5 projects are designed, implemented and evaluated yearly. Goals are specific to contracted work plans.
