Organizations Filed Purposes: THE MISSION OF MENTAL HEALTH RESOURCES IS TO FOSTER HOPE, HEALTH AND RECOVERY FOR PEOPLE AFFECTED BY MENTAL ILLNESS AND SUBSTANCE USE DISORDER (SUD).

TARGETED CASE MANAGEMENT (TCM) TEAMS PROVIDED PERSON-CENTERED SUPPORT TO ADULTS AND CHILDREN WHO HAVE BEEN DIAGNOSED WITH A SERIOUS AND PERSISTENT MENTAL ILLNESS. MHR PROVIDES PERSONALIZED SERVICE FOR INDIVIDUALS IN THE COMMUNITY WHO MAY HAVE CO-OCCURRING MEDICAL, SOCIAL AND SUBSTANCE ABUSE CHALLENGES. TCM CASE MANAGERS HELP CLIENTS GAIN ACCESS TO SERVICES NEEDED TO MEET THEIR MENTAL HEALTH NEEDS, ACHIEVE RECOVERY, AND LIVE INDEPENDENTLY IN THE COMMUNITY. SERVICES ARE PROVIDED BY SKILLED MENTAL HEALTH PRACTITIONERS WHO ARE SUPPORTED BY A TEAM LEADER AND A CLINICAL SUPERVISOR. CASE MANAGERS WORK CLOSELY WITH EACH INDIVIDUAL TO CREATE A TREATMENT PLAN, BASED ON ASSESSMENT OF FUNCTIONING, NEEDS AND STRENGTHS. WE MEET WITH INDIVIDUALS 1-4 TIMES PER MONTH IN THEIR HOME OR IN THE COMMUNITY. IN 2020, 2,617 CLIENTS WERE SERVED.

MHR ADMINISTERS HOUSING VOUCHERS IN THE 7-COUNTY METRO AREA FOR INDIVIDUALS LIVING WITH MENTAL ILLNESS, CHEMICAL DEPENDENCY AND OTHER DISABILITIES. THESE RENTAL ASSISTANCE SUBSIDIES ARE FROM A VARIETY OF SOURCES INCLUDING HUD SHP (RAMSEY, ANOKA, DAKOTA, AND HENNEPIN COUNTIES), BRIDGES, SECTION 8, HOUSING TRUST FUND AND RAMSEY SUPPORTIVE HOUSING VOUCHER PROGRAM. IN 2020, 1,261 CLIENTS WERE SERVED.

ASSERTIVE COMMUNITY TREATMENT (ACT) IS A SERVICE MODEL PROVIDED BY COMMUNITY- BASED, MOBILE MENTAL HEALTH TREATMENT TEAMS. THE ACT TEAM APPROACH IS DESIGNED TO PROVIDE COMPREHENSIVE PSYCHIATRIC TREATMENT, REHABILITATION, AND SUPPORT TO PERSONS WITH SERIOUS AND PERSISTENT MENTAL ILLNESS, OR PERSONALITY DISORDERS WITH SEVERE FUNCTIONAL IMPAIRMENTS, TO LIVE INDEPENDENTLY IN THE COMMUNITY. PERSONS SERVED BY ACT OFTEN HAVE CO- EXISTING PROBLEMS SUCH AS HOMELESSNESS, SUBSTANCE ABUSE, FREQUENT HOSPITALIZATION, AND/OR INVOLVEMENT WITH THE JUDICIAL SYSTEM. PEOPLE TYPICALLY SERVED BY ACT TEAMS OFTEN HAVE NEEDS THAT HAVE NOT BEEN EFFECTIVELY ADDRESSED BY TRADITIONAL, NON-INTENSIVE SERVICES. TEAM MEMBERS WORK WITH PEOPLE IN THEIR HOMES, WORK SETTING, OR PLACES IN THE COMMUNITY WHERE ADDITIONAL SUPPORT MIGHT BE NEEDED. IN 2020, 211 CLIENTS WERE SERVED.
