To improve the health of low-income people who face the greatest barriers to care access.
Vans/other clinics: the organization provides medical care to low-income, uninsured residents of Montgomery County, Maryland at little or no charge, as well as to Medicaid and low-income Medicare enrollees. The program is designed to provide access to quality health care to all residents age 18 and over who face significant financial barriers and have no third-party reimbursement available. Patients are seen regardless of their ability to pay. The majority of patients are between 30 and 65 years of age. Clinics are conducted at various locations - at fixed sites (Briggs Chaney, Rockville), in mobile medical vans (Aspen Hill, Gaithersburg, Long Branch), and two homeless shelters - at scheduled times during each week. The clinics are staffed primarily by paid nurse practitioners, nurses, support personnel, contracted therapists, and volunteer physicians. Patients are provided services on an appointment or walk-in basis which removes barriers around accessibility. The Organization continues to recruit volunteers for both professional and support staff positions to assist in the delivery of services. Mobile van clinics were temporarily suspended due to the pandemic.
Upcounty: the Upcounty Clinic in Germantown serves over 30% of the organization's total caseload, with ongoing lease support from Adventist HealthCare. This six-exam room clinic, open five days per week, provides a full array of primary care services, including behavioral health. Activities include regularly scheduled clinic sessions targeting the Asian immigrant community.
Head Start: The Organization contracts with the Montgomery County Department of Health and Human Services to provide health management services to children three to five years old who are enrolled in the Head Start and Pre-Kindergarten programs. Case management services and health screenings are offered to all Head Start/Pre-K program children and their families. In the process of assessing the needs of children, parents in need of primary care may be identified and referred to the Organization, if they have no insurance. During the 2019-2020 school year, the Organization provided services to children in about 80 classes in 35 Montgomery County Public Schools and community-based centers.
