HOSPICE OF THE CHESAPEAKE INC.'S MISSION IS TO IMPROVE THE QUALITY OF LIFE FOR THOSE IN OUR COMMUNITIES EXPERIENCING ADVANCED ILLNESS OR BEREAVEMENT THROUGH HOSPICE AND OTHER PALLATIVE CARE, COMPASSIONATE SUPPORT AND EDUCATION.
TO PROVIDE PHYSICAL, PSYCHO-SOCIAL AND SPIRITUAL SUPPORT SERVICES TO INDIVIDUALS LIVING WITH AND AFFECTED BY ADVANCED ILLNESS TO IMPROVE THE QUALITY OF THEIR LIVES. THESE SERVICES ARE PROVIDED WHEREVER THE PATIENT CALLS HOME. WE ARE COMMITTED TO PROVIDING THE BEST POSSIBLE CARE FOR ALL REGARDLESS OF THEIR INABLITY TO PAY. ADDITIONALLY, GRIEF SUPPORT SERVICES ARE AVAILABLE TO ALL MEMBERS OF THE COMMUNITY WHO HAVE LOST A LOVED ONE INCLUDING INDIVIDUALS WHO HAVE EXPERIENCED TRAUMATIC OR SUDDEN LOSS. USING A TEAM ORIENTED APPROACH INVOLVING OUR PHYSICIANS, NURSES, SOCIAL WORKERS, HOME HEALTH AIDS, CHAPLAINS AND COUNSELORS ALLOWS US TO ENSURE QUALITY AND COMPASSIONATE CARE. OUR SERVICES ARE AVAILABLE TO INDIVIDUALS ACROSS THE LIFE SPAN INCLUDING PERINATAL SUPPORT AND PEDIATRIC HOSPICE CARE.
HOSPICE PROGRAM - AN INTERDISCIPLINARY TEAM OF PHYSICIANS, NURSES, SOCIAL WORKERS, CERTIFIED NURSING ASSISTANTS, COUNSELLORS, CHAPLAINS AND SPECIALLY TRAINED VOLUNTEERS WORK WITH THE HOSPICE ELIGIBLE PATIENTS AND THEIR CAREGIVERS TO COORDINATE A PLAN OF CARE. AMONG ITS MAJOR RESPONSIBILITIES, THE INTERDISCIPLINARY HOSPICE TEAM MANAGES THE PATIENT'S PAIN AND SYMPTOMS; ASSISTS THE PATIENT WITH THE EMOTIONAL AND PSYCHOSOCIAL AND SPIRITUAL ASPECTS OF DYING; PROVIDES NEEDED DRUGS, MEDICAL SUPPLIES, AND EQUIPMENT; COACHES THE FAMILY ON HOW TO CARE FOR THE PATIENT; AND PROVIDES BEREAVEMENT CARE AND COUNSELLING TO SURVIVING FAMILY AND FRIENDS. HOSPICE CARE IS PROVIDED FROM BIRTH TO DEATH REGARDLESS OF AGE, RACE OR ETHNICITY.
PATIENT CARE CENTERS - PROVIDES CARE, ROOM AND BOARD TO ELIGIBLE HOSPICE PATIENTS IN RESIDENTIAL AND GENERAL INPATIENT ENVIRONMENTS.
CHESAPEAKE PALLIATIVE MEDICINE - SUPPORTS HOSPICE PROGRAM WITH PHYSICIAN'S SERVICES AND PROVIDES PALLIATIVE MEDICINE CONSULTATIONS AND CARE TO THOSE LIVING WITH COMPLEX ILLNESS.
