THE JEWISH FOUNDATION FOR GROUP HOMES IS COMMITTED TO PROVIDING INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES AND/OR CHRONIC MENTAL DISORDERS WITH THE OPPORTUNITY TO LIVE INDEPENDENTLY WITHIN THE COMMUNITY WITH DIGNITY, PERSONAL CHOICE AND RESPECT WHILE OFFERING THE OPPORTUNITY TO EXPERIENCE JEWISH LIFE AND SUPPORTING OTHERS WITH THE OPPORTUNITY TO PRACTICE THEIR OWN FAITH.
PROVIDE INDIVIDUALS WITH I/DD OPPORTUNITIES TO CHOOSE WHERE THEY LIVE AND RECEIVE SUPPORTS.
GROUP HOMES AND ALTERNATIVE LIVING UNITS: COMMUNITY LIVING SUPPORTS PROVIDED IN RESIDENTIAL SETTINGS OWNED OR LEASED BY JFGH. EACH HOME HAS BETWEEN 1 AND 6 INDIVIDUALS RECEIVING SUPPORTS. SUPPORTS INCLUDE, BUT ARE NOT LIMITED TO ASSISTANCE WITH PERSONAL NEEDS, HOUSEHOLD CHORES, FOOD SHOPPING AND PREPARATION, TRANSPORTATION, RECREATIONAL ACTIVITIES AND HEALTH AND MEDICAL MONITORING.JFGH HAS 4 INDIVIDUALS IN A ONE-PERSON GROUP HOME, 8 INDIVIDUALS WHO SHARE A 2 PERSON GROUP HOME, 9 RECEIVING SUPPORTS IN A 3-PERSON HOME, 56 IN A 4-PERSON HOMES, 20 IN A 5-PERSON HOMES AND 12 INDIVIDUALS RECEIVING SUPPORTS IN 6-BEDROOM HOMES.
IN-HOME SUPPORTS: SUPPORTS TO INDIVIDUALS WITH DISABILITIES AND THE FAMILIES WHO OWN OR RENT THEIR OWN RESIDENTIAL DWELLING. JFGH PROVIDES AND MANAGES SUPPORT STAFF AND COUNSELING SERVICES IN ACCORDANCE WITH THE CLIENT'S NEEDS. SUPPORTS INCLUDE, BUT ARE NOT LIMITED TO, PERSONAL NEEDS, HOUSEHOLD CHORES, FOOD SHOPPING AND TRANSPORTATION, RECREATIONAL ACTIVITIES, AND HEALTH AND MEDICAL MONITORING.DURING FY2020, JFGH PROVIDED IN-HOME SUPPORT SERVICES TO 71 INDIVIDUALS.
MEANINGFUL OPPORTUNITIES FOR SUCCESSFUL TRANSITIONS (MOST): A ONE-YEAR INDIVIDUALIZED PROGRAM OF ACTIVITIES AND TRAINING DESIGNED TO FACILITATE SUCCESSFUL TRANSITION OF YOUNG ADULTS WITH DISABILITIES INTO THE ADULT COMMUNITY. THE PROGRAM SERVES AS A PORTAL FOR INDIVIDUALS AND THEIR FAMILIES, THROUGH WHICH THEY ARE INTRODUCED AND INTEGRATED INTO A BROAD RANGE OF COMMUNITY RESOURCES. THE PROGRAM FOCUSES ON FACILITATING OUTCOMES FOR PARTICIPANTS IN THREE DOMAINS INCLUDING SELF-SUFFICIENCY (INDEPENDENT LIVING AND SAFETY SKILLS), COMMUNITY (SOCIAL SKILLS, TRANSPORTATION AND RECREATION), AND VOCATION (EMPLOYMENT, EDUCATION, VOCATIONAL TRAINING, DAY HABILITATION).EACH OF THE THREE LOCATIONS FOR THE MOST PROGRAM CAN ACCOMMODATE UP TO 15 PARTICIPANTS RECEIVING SUPPORTS ON A 3:1 PARTICIPANT TO STAFFING RATIO EXCEPT FOR THOSE WHO NEED 1:1 SUPPORTS.
